不同类型精神分裂症患者治疗前后甲状腺功能指标的比较分析

Comparative analyses of thyroid hormone levels before and after the treatment of schizophrenic patients of different symptomtypes

目的:对照阳性和阴性症状为主的精神分裂症患者治疗前、后血清甲状腺激素水平的变化,探讨甲状腺功能与精神分裂症的相关性。方法:将60例首发精神分裂症患者作为研究组;选取健康体检人员45例作为对照组。治疗前、后,检测研究组患者的血清甲状腺激素水平并与正常对照组比较;并利用PANSS量表将研究组患者又分为P组和N组,每组各30例。P组患者为阳性症状亚组;N组患者为阴性症状亚组。分析两组研究对象相关因素变化的相关性。结果:治疗后,P组患者的FT_4和FT_3血清水平较治疗前降低,且均低于对照组;治疗前,N组患者的FT_3血清水平明显高于P组。结论:抗精神病药物能引起精神分裂症患者血清甲状腺激素水平的变化。因此,临床中应重视精神分裂症患者伴发甲状腺功能减退危险因素的评估,定期监测其甲状腺激素水平并尽早采取干预措施。

Objective： To compare changes of thyroid hormone levels of schizophrenic patients with positive and negative symptoms before and after the treatment, and discuss correlations of thyroid function and schizophrenia. Methods： 60 schizophrenic- patients were selected as study group, and45 healthy people for physical examination were used as control group. The thyroid hormone levels of the study group before and after the treatment were detected, and then the study group was subdivided into P group （n = 30, positive symptom subgroup） and N group （n = 30, negative symptom subgroup） according to the scores of PANSS. The related factors of the changes of the two groups were analyzed. Results ： The serum FT4 and FT3 levels after the treatment of the study group were lower than those before the treatment, and the values before and after the treatment were both lower than those of control group. Before the treatment, the serumFT3 levels of N group were significantly higher than those of P group. Conclusions ： Antipsychotic drugs can cause the thyroid hormone levels change for the schizophrenic patients. Therefore,it should pay attention to assess the dangerous factors of schizophrenia complicated with hypothyroidism in the clinic, monitor the thyroid hormone levels regularly and adopt intervention measures as early as possible.
