消化道恶性肿瘤腹壁侵袭转移的外科治疗策略与选择 被引量:2

The surgical strategies for abdominal wall invasion and metastasis of digestive malignant tumors

摘要 消化道恶性肿瘤腹壁侵袭转移的发病率低,预后差,严重影响患者的生存与生活质量。随着现代诊断与治疗技术的进步,特别是植入材料技术、整复重建外科技术以及组织结构分离技术的推广与应用,实施腹壁肿瘤的扩大切除并在此基础上进行腹壁重建成为可能,消化道恶性肿瘤的预后得到了显著改善,带瘤患者生存时间显著延长。充分的术前评估与准备、多学科合作及合理的术式选择与实施是患者得到有效、正确治疗的保证。 The incidence of invasion and metastasis to abdominal wall from the digestive malignant tumor which is regarded as the signal of tumor spread is low. These patients usually accepted conservative treatment and their prognosis was poor. With the development of modem medical concepts and technology, resection rate of the digestive tract tumor obviously increased and the patient survival get longer. A growing number of reports showed that the surgical treatment could prolong the survival time of patients with invasion and metastasis to abdominal wall from the digestive malignant tumor, decrease the complications caused by the abdominal tumor, and improve quality of life. The large abdominal wall defect after tumor resection could be reconstructed by joint technologies, such as mesh, component separation technique and autologous tissue transplantation.
