桂东地区258例泌尿系结石患者的结石成分与相关危险因素分析 被引量:17

摘要 目的:分析桂东地区泌尿系结石的化学成分与发病的相关危险因素,为该地区泌尿系结石的防治提供依据。方法:收集桂东地区2015年1月~2016年10月经泌尿外科手术或体外冲击波碎石的泌尿系结石患者258例为研究组,采用红外光谱法分析其结石的化学成分;另选同期于我院泌尿外科住院的非结石患者160例为对照组,对两组患者的16个相关变量进行单因素分析与多因素Logistic回归分析。结果:在258例结石样本中,含草酸钙的结石最多,检出率高达91.86%;其次为含羟基磷灰石的结石(23.64%)与含尿酸的结石(19.77%)。Logistic回归分析表明日饮水量<1L、喝浓茶≥1次/d、饮酒≥2次/周、常食用动物内脏、精神紧张度高、有家族史是该地区泌尿系结石的独立危险因素。结论:桂东地区泌尿系结石的化学成分以草酸钙结石为主;发病主要受日饮水量少、喜喝浓茶、喜饮酒、常食用动物内脏、精神紧张度高和有家族史等因素的影响。 Objective:To analyze the stone composition and related risk factors in urinary calculi patients in Guidong and to provide evidence for the prevention and treatment of urinary calculi in this area.Method:From January 2015 to October 2016,258 urinary calculi patients treated with operation or extracorporeal shock wave lithotripsy in Guidong were chosen as research group,then the stone composition was analyzed by infrared spectroscopy.Other 160 patients without urinary calculi were chosen as controlled group.Then single factor analysis and multi-factor logistic regression analysis were performed on 16 related variables in two groups of patients.Result:In 258 cases of stone samples,calcium oxalate stone was the most(91.86%),followed by the hydroxyapatite containing stones(23.64%)and uric acid stones(19.77%).Logistic regression analysis showed that daily water intake〈1L,drinking strong tea ≥ 1times/day,drinking≥2times/week,often eating animal organs,mental tension and family history of stone were independent risk factors of urinary calculi in this region.Conclusion:The calcium oxalate is the main chemical composition of urinary stones in Guidong and the incidence of urinary stones is mainly affected by low daily water intake,more drinking tea,more drinking,often eating animal organs,mental tension and family history of stone.
