腰椎压缩性骨折复位枕放射学参数研究 被引量:1

摘要 目的观察腰1椎体压缩性骨折复位枕弧面放射学参数,为临床胸腰椎压缩性骨折复位枕设计提供临床参考依据。方法选取该院2014年4月‐2016年4月住院的80例腰1椎体压缩性骨折患者为研究对象,采用弧面复位枕治疗前、治疗后第2、4和12周行X线摄片检查,统计分析椎体压缩比、后凸Cobb角及椎体棘突末端离床面垂直距离。结果腰1椎体压缩性骨折患者治疗后第2、4和12周椎体压缩比、后凸Cobb角分别为(10.85±9.1)%、(11.09±8.78)%、(9.15±4.64)%、(5.15±5.05)°、(5.41±4.98)°和(4.15±1.7)°,均显著低于治疗前的(22.46±10.21)%、(10.16±4.5)°(P<0.05);腰1椎体压缩性骨折患者治疗后第2、4和12周椎体棘突末端离床面垂直距离分别为(5.93±2.71)mm、(5.52±2.11)mm和(8.09±4.29)mm,均显著高于治疗前(3.44±2.19)mm(P<0.05);腰1椎体压缩性骨折患者治疗后第12周椎体压缩比、后凸Cobb角均低于第2、4周(P<0.05);腰1椎体压缩性骨折患者治疗后第12周椎体棘突末端离床面垂直距离均高于第2、4周(P<0.05)。结论通过放射学研究显示,腰1椎体压缩性骨折患者通过高度为5~11 cm弧面复位枕渐进式抬高复位治疗,放射学参数有显著改善,可以将其作为胸腰椎压缩骨折复位枕设计参数的理论依据。 [Objective]To observe the radiologic parameters of the surface of reposition pillow for lumbar vertebral body compression fractures,and to provide clinical references for the design of reduction pillow for clinical thoracolumbar compression fracture.[Methods]Eighty patients with lumbar vertebral compression fractures who were hospitalized in our hospital from April2014 to April 2016 were selected as the research objects,and the X-ray examination was performed at the 2nd,4th and 12 th week before and after treatment by the arc reduction pillow.Then,the vertebral compression ratio,kyphosis Cobb angle and the vertical distance from the spinous process of vertebra were statistically analyzed.[Results]At the 2nd,4th and 12 th week after treatment,the vertebral compression ratios,kyphosis Cobb angles of patients with lumbar vertebral compression fracture respectively were(10.85±9.1)%,(11.09±8.78)%,(9.15±4.64)%,(5.15±5.05)°,(5.41±4.98)° and(4.15±1.7)°,which were significantly lower than(22.46±10.21)%and(10.16±4.5)° before treatment(P 0.05);at the 2nd,4th and 12 th week after treatment,the vertical distances from the spinous process of vertebral body of patients with lumbar vertebral compression fracture respectively were(5.93±2.71) mm,(5.52±2.11) mm and(8.09±4.29) mm,which were significantly higher than(3.44±2.19) mm before treatment(P 0.05);the vertebral compression ratio,kyphosis Cobb angle at 12 th week after treatment were lower than those at the 2nd and 4th week(P 0.05);the vertical distance from the spinous process to the bed surface was higher than that at the 2nd and 4th week(P 0.05).[Conclusion]The radiological research shows that after the treatment of progressive elevation reset with arc pillow of 5~11 cm height,the radiographic parameters of patients with lumbar vertebral compression fracture are significantly improved,thus it could be used as the theoretical basis for the design of the reduction pillow.
