芪苈强心胶囊对慢性心功能不全患者额面QRS-T夹角的影响研究

Influence of Qiliqiangxin Capsule on QRS-T Angle in Chronic Heart Failure Patients

目的探讨芪苈强心胶囊对慢性心功能不全(chronic heart failure,CHF)患者额面QRS-T夹角的影响。方法连续性纳入2015-01/2016-01月于作者医院就诊的130例CHF患者,随机均分为芪苈强心胶囊组(n=65)和对照组(n=65)。分别于入组前和治疗后6个月进行6 min步行试验、超声心动图检查、N末端脑钠肽前体(N-terminal proB-type natriuretic peptide,NT-proBNP)检测和QRS-T夹角计算。结果经过6个月治疗,芪苈强心胶囊组患者额面QRS-T夹角明显下降(P<0.05)且低于同期对照组患者水平(P<0.05),左室射血分数(left ventricular ejection fraction,LVEF)明显升高(P<0.05)且大于同期对照组患者水平(P<0.05),NT-proBNP水平明显降低(P<0.05)且低于同期对照组患者水平(P<0.05),6 min步行距离明显增加(P<0.05)且高于同期对照组患者水平(P<0.05)。芪苈强心胶囊干预治疗与△额面QRS-T夹角(r=-0.669,P<0.05)和△NT-proBNP呈负相关(r=-0.641,P<0.05),与△LVEF呈正相关(r=0.546,P>0.05)。△额面QRS-T夹角与△NT-proBNP呈正相关(r=0.622,P<0.05),与△LVEF呈负相关(r=-0.526,P>0.05)。结论芪苈强心胶囊可以明显降低CHF患者异常升高的额面QRS-T夹角。

Objective To evaluate the influence of Qiliqiangxin（QLQX）capsule on QRS-T angle in chronic heart failure（CHF）patients.Methods One hundred and thirty CHF patients were enrolled in this study and divided into two groups equally and randomly：QLQX group and control group.Six minutes walking test（6MWT）,cardiac ultrasound,levels of N-terminal pro-B-type natriuretic peptide（NT-proBNP）and QRS-T angle were measured at the beginning and 6months after treatment.Results In QLQX group,QRS-T angle and levels of NT-proBNP were decreased after 6 months treatment（P〈0.05）,while left ventricular ejection fraction（LVEF）and 6 MWT results were increased（P〈0.05）.QRS-T angle and levels of NT-proBNP in QLQX group at 6 months were significantly lower than those in control group（P〈0.05）.Correlation analysis showed that QLQX treatment was negatively related with △QRS-T（r=-0.669,P〈0.05）and △NT-proBNP（r=-0.641,P〈0.05）,and positively associated with △LVEF（r=0.546,P〈0.05）.Conclusion QLQX can reduce size of QRS-T angle in CHF patients.
