清热解毒汤治疗血热型寻常性银屑病的疗效观察及对血清TNF-α的影响

The Efficacy of QingReJieDu Decoction in the Treatment of Psoriasis Vulgaris with the Pattern of Blood-Heat and the Influence on Level of TNF-α in the Serum

目的观察清热解毒汤治疗血热型寻常性银屑病的临床疗效,探讨其对血清TNF-α的影响情况。方法将入选的60例患者随机分成两组,全部患者予外用维A酸哈西奈德乳膏,2次/d;治疗组(30例)同时予口服清热解毒汤,2次/d,所有患者的疗程均为4周。采用PASI评分评价治疗前后的临床疗效,酶联免疫吸附试验(ELISA)法检测治疗前后血清TNF-α水平。结果治疗结束后,治疗组有效率(80.0%)明显高于对照组有效率(30%),治疗组PASI评分和血清TNF-α水平均较其治疗前明显降低,且治疗组PASI评分(3.80±1.47)明显低于对照组PASI评分(9.62±1.97),以上差异均有统计学意义(P均<0.05)。结论清热解毒汤治疗血热型寻常性银屑病疗效显著,可能通过降低患者血清TNF-α水平发挥治疗作用。

Objective To observe the clinical efficacy of QingReJieDu decoction in the treatment of psoriasis vulgaris patients with the pattern of blood-heat and the influence on level of TNF-α in the serum. Methods All se- lected 60 patients were randomly divided into two groups. All patients were given topical tretinoin/halcinon- ide cream （twice a day）. Treatment group（ 30 cases） was treated with QingReJieDu decoction by oral admin- istration （twice a day） at the same time. All groups were treated for 4 weeks. The clinical efficacy was eval- uated by PASI score before and after treatment. Meanwhile, levels of TNF-α in serum were detected by en- zyme linked immunosorbent assay ELISA. Results After treatment, the effective rate of the treatment group （80. 0% ） was significantly higher than that of the control group （ 30% ） ; the PAS1 score and the level of TNF-α in the serum of treatment group were significantly decreased; and the PASI score of the treatment group（3.80 ± 1.47）was significantly lower than that of the control group（9.62 ± 1.97）. All of these differ- ences were statistically significant compared with control group （ P 〈 0. 05 ）. Conclusion QingReJieDu de- coction in the treatment of psoriasis vulgaris patients with the pattern of blood-heat is effective, possibly by reducing expression level of TNF-α in the serum.
