摘要 目的 :探讨严重多发伤患者血清肌红蛋白(Mb)、降钙素原(PCT)与C-反应蛋白(CRP)水平检测及临床意义。方法 :选取2014年3月~2015年3月我院收治的80例严重多发伤患者作为研究对象,根据患者感染情况分为感染组与未感染组,感染组43例,未感染组37例,另选取同期在我院健康体检40例作为对照组,分别使用免疫比浊法测定血清肌红蛋白、半定量快速法测定降钙素原、散射比浊法测定C-反应蛋白的水平,并将三组检测结果进行对比分析。结果 :感染组与为感染组患者从年龄、性别、受伤原因进行对比,两组间无明显差异,差异无统计学意义(P>0.05)。三组受试者在入院后在不同时间点Mb、PCT及CRP水平对比有显著差异,差异有统计学意义(P<0.05);感染组与未感染组Mb、PCT及CRP水平进行对比有显著差异,差异有统计学意义(P<0.05);随着入院时间的增长,感染组与未感染组的Mb、PCT水平呈持续增长趋势,在入院后3~5d时增长最快,随后时间的延长,增长速度有所变慢。结论:临床检测血清肌红蛋白(Mb)、降钙素原(PCT)与C-反应蛋白(CRP)水平能为判断严重多发伤患者的伤情提供准确的诊断依据,值得临床应用。 Objective: To investigate the levels of serum myoglobin (Mb), procalcitonin (PCT) and C-reactive protein (CRP) in patients with severe multiple trauma and their clinical significance. Methods: 80 patients with severe multiple injuries admitted to our hospital from March 2014 to March 2015 were enrolled in this investigation. The patients were divided into infection group and non infection group according to the situation of infection, 43 cases in the infection group, and 37 cases in the uninfected group. Another 40 healthy patients in our hospital were selected as control group, Serum myoglobin was measured by immunoturbidimetric assay, and procalcitonin was determined by semi quantitative rapid determination. The level of C-reactive protein was determined by turbidimetric method. Test results of the three groups were compared and analyzed. Results: No significant difference was detected between the infection group and the infection group in terms of age, sex and cause of injury (P〉0.05). The levels of Mb, PCT and CRP in the three groups were significandy different after admission (P〈0.05). Significant difference was detected in the levels of Mb, PCT and CRP between the infected group and the uninfected group at the different time points (P〈0.05). The Mb and PCT levels of the infected group and the uninfected group showed a trend of increasing with the increase of admission time, which was the fastest at 3-5 days after admission, Conclusion: The clinical detection of serum myoglobin (Mb), procalcitonin (PCT) and C-reactive protein (CRP) can provide an accurate diagnosis basis for judging the injury of patients with severe multiple injuries and deserves clinical application.
