茵陈蒿汤治疗非酒精性脂肪性肝炎的临床疗效及对TLR-4表达的影响

Clinical study on effect of Yinchenhao Decoction in treating nonalcoholic fatty liver Diseases and the influence of the expression of TLR-4

目的:观察茵陈蒿汤对非酒精性脂肪性肝炎的临床疗效以及对TLR-4表达的影响。方法:收集确诊为NASH患者80例,随机分为两组,治疗组患者口服茵陈蒿汤150ml/次,2次/d,对照组患者采用运动疗法,疗程均为两月。观察两组患者治疗前后中医证候、肝功能、血脂、血糖(GLU)及TLR-4的变化情况。结果:治疗组患者乏力、肝区不适、头晕、胸闷及口苦等症状改善情况优于对照组,差异有显著性意义(P<0.05)。治疗组中医证候疗效总有效率为85%,对照组为47.5%,两组比较差异有显著性意义(P<0.05)。治疗组患者在谷氨酰转移酶(GGT)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL)、空腹血糖(GLU)、总胆红素(TBil)及人体Toll样受体4(TLR-4)改善方面优于对照纽(P<0.05);在丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)变化方面比较,两组差异无显著性意义(P>0.05)。结论:茵陈蒿汤能有效改善NASH患者GGT、血脂、血糖及TLR4,并能明显改善患者中医证候,临床值得推广应用。

Objective ：To observe the clinical effect of Yinchenhao Decoction in treating nonalcoholic fatty liver diseases and the influence of the expression of TLR-4. Methods： Eighty patients with NAFLD were randomized into two groups. The patients in treatment group were treated by taking Yinchenhao Decoction, 150ml one time ,Twice a day; the control group were exercising. All these patients were followed up 2 months to observe the changes in symptom, liver function, blood lipid, GLU and TLR-4. Results ：The effective rate of the treatment group in alleviaring symptoms such as Anergy, indisposition in hepatic region, swirl, chest tightness or bitterness in the mouth was superior to the control group（ P 〈 0.05）. The total effective rate of TCM syndrome was 88.3% in treatment group, and 70% in the control group and there were significant differences in the total effective rates of TCM syndrome between two groups （P 〈 0.05）. The treatment group was superior to the control group in the aspects of lowering the levels of GGT, TC, TG, LDL, GLU, TBil and TLR-4 （P 〈 0.05 ）. There were no difference between two groups in ameliorating ALT and AST （ P 〉 0.05 ）. Conclusion： Yinchenhao Decoction could significantly improve GGT, TC, TG, LDL, GLU, TBil and TLR-4 and exhibit more effects jn ameliorating blood-lipid and alleviating symptoms in treating NAFLD.
