舒芬太尼联合丙泊酚麻醉在腹腔镜子宫肌瘤剔除术中的效果

Efficacy of sufentanil combined with propofol as anesthesia in laparoscopic myomectomy

目的探讨舒芬太尼联合丙泊酚麻醉在腹腔镜子宫肌瘤剔除术中的效果。方法选取2015年1月至2016年7月间在海南省海口市妇幼保健院行腹腔镜下子宫肌瘤剔除术治疗的124例子宫肌瘤患者为研究对象,采用掷硬币法随机分成观察组和对照组,每组62例。观察组患者采用舒芬太尼联合丙泊酚麻醉,对照组患者采用瑞芬太尼联合丙泊酚麻醉,对比不同麻醉下,两组患者腹腔镜子宫肌瘤剔除术治疗后的临床疗效以及不良反应发生情况。结果观察组患者的总有效率(98.4%)高于对照组患者的总有效率(72.6%),对照组患者的自主呼吸恢复时间(12.85±3.01)min、拔管时间(22.66±4.98)min、苏醒时间(14.02±1.55)min以及血浆黏度(7.37±0.89)m Pa.s多于观察组患者的自主呼吸恢复时间(7.22±3.08)min、拔管时间(15.88±3.98)min、苏醒时间(8.99±1.47)min以及血浆黏度(5.99±0.77)m Pa.s,且观察组患者术后并发症总发生率(3.2%)低于对照组患者术后并发症总发生率(29.0%),差异均有统计学意义(均P<0.05)。结论腹腔镜下子宫肌瘤剔除术患者术中采用舒芬太尼与丙泊酚联合麻醉的效果较好,值得在临床推广应用。

Objective To study the efficacy of sufentanil combined with propofol as anesthesia in laparoscopic myomectomy. Methods From January 2015 to July 2016,124 patients who underwent laparoscopic myomectomy at Haikou Maternity and Infant Health Hospital were selected as the research subjects.Using the coin toss method,patients were randomly divided into an observation group and a control group with 62 patients in each group. The observation group received sufentanil combined with propofol as anesthesia and the control group received remifentanil combined with propofol as anesthesia. Clinical efficacy and adverse reactions after laparoscopic myomectomy were compared. Results The overall efficiency of the observation group 98. 4% was higher than that of the control group 72. 6%. Time to spontaneous respiration,time to extubation,time to recovery and plasma viscosity was（ 12. 85 ± 3. 01） min,（ 22. 66 ± 4. 98） min,（ 14. 02 ± 1. 55） min and（ 7. 37 ± 0. 89） m Pa. s respectively in the control group which was significantly more than（ 7. 22 ± 3. 08） min,（ 15. 88 ± 3. 98） min,（ 8. 99 ± 1. 47） min and（ 5. 99 ± 0. 77） m Pa. s of the observation group. The overall incidence of complications in the observation group（ 3. 2%） was significantly lower than that of the control group（ 29. 0%）（ P〈0. 05）. Conclusion The combination of sufentanil and propofol showed good anesthesia effects in patients undergoing laparoscopic myomectomy,which is worth being clinically widely used.
