整体护理对膀胱全切术行泌尿造口患者生活质量的影响

Effect of holistic nursing on quality of life in patients undergoing bladder cancer surgery

目的探讨整体护理对膀胱全切术行泌尿造口患者生活质量的影响。方法选取2013年4月至2015年5月间四川省彭州市人民医院收治的120例行膀胱全切泌尿造口治疗的膀胱肿瘤患者,采用随机数表法分为观察组和对照组,每组60例。观察组患者在常规护理基础上行整体护理,对照组患者行常规护理。比较两组患者治疗依从率、生活质量、掌握自我护理的能力和造口周围皮肤受损情况。结果观察组患者依从率为91.7%,明显高于对照组患者的73.3%,差异有统计学意义(P<0.05)。观察组患者造口周围皮肤无损害例数占98.3%,明显高于对照组患者的80.0%,90.0%的观察组患者掌握护理知识良好,远高于对照组患者的35.0%,差异有统计学意义(P<0.05);观察组患者身体功能、角色功能、情绪功能、认识功能、社会功能和总体健康状态得分均高于对照组患者,差异均有统计学意义(均P<0.05)。结论整体护理有助于肿瘤手术行泌尿造口患者提高依从率,减少患者造口周围皮肤感染情况,提高患者的生活质量。

Objective To explore the effect of holistic nursing on the quality of life in patients undergoing bladder cancer surgery. Methods One hundred and twenty patients undergoing surgery of urinary stoma for bladder cancer were selected at Pengzhou People＇s Hospital from April 2013 to May 2015. According to the random number table,patients were divided into an observation group and a control group with 60 patients in each group. Patients in the observation group received holistic nursing and patients in the control group received routine operation nursing. Compliance rate,quality of life,the ability to grasp the skill of self-care and peri-stoma skin damage were observed and compared between the two groups. Results Compliance rate was 91. 7% for the observation group which was significantly higher than 73. 3% of the control group（ P〈0. 05）. Non-peri-stoma skin damage was found in 98. 3% patients in the observation group which was higher than 80. 0% of the control group. 90. 0% patients had good grasp of nursing knowledge in the observation group which was much higher than 35. 0% of the control group（ P〈0. 05）. The score of physical function,role function,emotional function,cognitive function,social function and the overall state of health in the observation group was higher than in the control group（ all P〈0. 05）. Conclusion Holistic nursing intervention is helpful to improve the compliance rate,reduce the incidence of peri-stoma skin infection and improve the quality of life in patients undergoing bladder cancer surgery.
