后腹腔镜与开放性离断式肾盂成形术治疗肾盂输尿管连接部梗阻对比研究

Comparative study of therapeutic effects of retroperitoneal laparoscopic and open retroperitoneoscopic Anderson-Hynes dismembered pyeloplasty in the treatment of ureteropelvic junction obstruction

目的分析采用后腹腔镜与开放性离断式肾盂成形术治疗肾盂输尿管连接部梗阻患者的效果。方法选取2013年1月至2015年12月在濮阳市人民医院治疗肾盂输尿管连接部梗阻的70例患者,随机分为观察组与对照组,各35例,对照组采用开放性离断式肾盂成形术,观察组采用后腹腔镜下离断式肾盂成形术,观察两组手术情况、术后恢复情况、并发症发生率。结果观察组术中出血量、手术时间、住院时间、肠胃功能恢复时间、下床活动时间均明显优于对照组(P<0.05);两组并发症发生率差异无统计学意义(P>0.05)。结论对肾盂输尿管连接部梗阻患者采用后腹腔镜下离断式肾盂成形术治疗效果显著,可有效缩短手术时间及术后恢复时间,有助于患者术后功能恢复,且安全性高。

Objective To analyze the therapeutic effects of retroperitoneal laparoscopic and open retroperitoneoscopic Anderson-Hynes dismembered pyeloplasty in the treatment of patients with ureteropelvic junction obstruction.Methods Seventy patients with ureteropelvic junction obstruction treated in People＇s Hospital of Puyang from January of 2013 to December of 2015 were divided into observation group and control group, 35 cases in each group.The control group was treated with open pyeloplasty, while the observation group was treated with retroperitoneal laparoscopic pyeloplasty.Patients of two groups were observed in the operation and postoperative recovery and the incidence of complications.Results The amount of bleeding, operation time, length of stay, recovery time of gastrointestinal function and the time of bed activity in the observation group were significantly better than those in the control group （P〈0.05）.The incidence of comlications between two groups had no statistically significant （P〉0.05）.Conclusion The therapeutic effect of retroperitoneal laparoscopic pyeloplasty is remarkable in the treatment of patients with ureteropelvic junction obstruction.It can effectively shorten the operation time and recovery time, and is helpful for the functional recovery of the patients with a certain security.
