哮喘合并变应性鼻炎患儿呼出气一氧化氮与鼻一氧化氮的关系 被引量:3

摘要 【目的】探讨支气管哮喘合并变应性鼻炎(AR)患儿上、下气道炎症相关性,为精准医疗提供理论依据。【方法】选取2015年1月至2016年6月本院门诊及住院收治的咳嗽和哮喘患儿169例作为研究对象,其中支气管哮喘合并AR患儿72例(A组),咳嗽变异性哮喘患儿55例(B组),非变应反应咳嗽患儿42例(C组),选择同期健康体检儿童34例为对照组(D组)。检测所有受试者呼出气一氧化氮(fractional exhaled ni-tricoxide,FeNO),鼻一氧化氮(nasal nitric oxide,NN0)和外周血嗜酸性粒细胞(EOS)%的变化;同时检测各组患儿的血常规。【结果】A组和B组的FeN0,NN0和EOS%水平明显高于C组和D组。差异有统计学意义(P〈O.05);A组FeNO,NN0和EOS%水平又高于B组(P〈O.05);C组与D组之间FeN0,NN0和EOS%水平比较差异无显著性意义(P〉0.05)。高FeN0、NNO水平患儿中A组所在的百分比明显高于B组(P〈O.05),而D组与D组之间比较差异无统计学意义(P〉0.05)。FeNO的ROC曲线下面积为0.855,95%可信区间为(O.788,O.921),约登指数最大值为O.686,灵敏度为72.2%,特异度为96.4%;NN0的ROC曲线下面积为O.850,95%可信区间为(O.781,O.920),约登指数最大值为0.610,灵敏度为79.2%,特异度为81.8%。A组与B组的FeN0,NN()和EOS%存在显著相关性(r=O.567,O.714,P〈O.01);FeNO与NNO之间也存在显著相关性(r=O.715,P〈0.01)。【结论】FeN()与NN0密切相关,监测FeN0与NN0对支气管哮喘合并AR患儿诊治有临床意义。
