饮酒量与糖尿病早期肾脏疾病相关性研究 被引量:2

摘要 目的:探讨不同饮酒量与糖尿病早期肾脏疾病的相关性。方法:对2012年2月-2015年6月在我院内分泌科住院及门诊的600例2型糖尿病患者进行调查。糖尿病早期肾脏疾病诊断以30mg≤24h尿微量白蛋白定量<300mg且肾功能正常(血肌酐<133μmol/L)为标准。按照患者饮酒量进行分组,比较大量饮酒组、适量饮酒组、不饮酒组的基础信息和糖尿病早期肾脏疾病患病率。结果:三组糖尿病患者在年龄、体重指数、糖尿病抵抗指数、空腹血糖、糖化血红蛋白差异无统计学意义。早期肾脏疾病患病率大量饮酒组为70.2%,适量饮酒组为27.3%,不饮酒组为42.8%;大量饮酒组早期肾脏疾病的患病率明显高于不饮酒组和适量饮酒组,适量饮酒组早期肾脏疾病患病率低于不饮酒组。结论:酒精的适量摄入可能是糖尿病早期肾脏疾病患病率降低的一个重要因素,而过量摄入则可能是导致糖尿病早期肾脏疾病患病率增加的一个重要因素。 Objective: To investigate the relationship of alcohol consumption with early diabetic kidney disease(ED KD). Methods:Our hospital endocrinology department from February 2012 to June 2015,600 cases type 2 diabetic patients accepted information inqury, physical examination and blood sample detection. Early diabetic kidney disease was defined as 30mg≤24h microalbuminuria 〈 300mg and normal renal function (serum creatinine 〈133μmol/L). All the study patients were classified into heavy alcohol consumption group, moderate alcohol consumption group, and no alco- hol consumption group. Comparison of basic information between three groups and prevalence of early diabetic kidney disease by SPSS19.0. Results: There was no significant difference in age, body mass index, diabetes resistance index, fasting blood glucose and glycosylated hemoglobin between the three groups. 118 patients(70. 2%)were found to have EDKD in heavy alcohol consumption group, and 12 patients(27.3%) in moderate alcohol consumption group, 166 patients(42.8 %)in no alcohol consumption group. The analysis of chi square test showed that mobidity of EDKD in heavy alehol consumption group is significantly greater than that of in moderate alcohol consumption group and in no alcohol consumption group, mobidity of EDKD in no alcohol consumption group is significantly greater than that of in moderate alcohol consumption group. Conclusion: Adequate intake of alcohol may be an important factor in reducing the preva lence of early kidney disease in diabetes,and excessive intake may be an important factor leading to an increased prevalence of early kidney disease in diabetes.
