摘要
目的:研究并探讨糖原磷酸化酶同工酶脑型联合其他标志物检测在窒息新生儿心肌损伤中的应用价值。方法:选取2012年6月—2015年6月我院收治的100例窒息新生儿分为心肌损伤组、非心肌损伤组,另选取同期50例正常新生儿作为对照组,比较三组新生儿的糖原磷酸化酶同工酶脑型(GPBB)、血浆N端脑钠肽(NT-proBNP)、缺血修饰白蛋白(IMA)检测,并比较单项检测与三项联合检测对新生儿窒息合并心肌损伤的诊断结果。结果:心肌损伤组新生儿的GPBB、NT-proBNP、IMA水平远远高于非心肌损伤组、对照组(P<0.05);三项标志物联合检测对新生儿窒息合并心肌损伤的诊断敏感性、特异性、准确性均高于单项检测(P<0.05)。结论:GPBB、NT-proBNP、IMA等标志物可对窒息新生儿的心肌损伤情况予以反映,三项标志物联合检测在窒息新生儿的心肌损伤诊断中具有显著的诊断价值。
Objective: To study and explore glycogen phosphorylase isoenzyme BB in combination with other markers in the application value of myocardial injury in neonatal asphyxia. Methods: From June 2012 to June 2015 in our hospi- tal 100 cases of neonatal asphyxia were divided into myocardial injury group and non myocardial injury group, the other selected a total of 50 cases of normal newborns as control group,glycogen phosphorylase isoenzyme BB three neonatal group comparison (GPBB),blood plasma N terminal brain natriuretic peptide (NT-proBNP), ischemia modified albu- min (IMA) detection,and compare the diagnosis results of single detection and combined detection of three asphyxia complicated with myocardial injury in newborns. Results: The levels of NT-proBNP, IMA, neonatal GPBB in Myocardial injury group is significantly higher than the non myocardial injury group and control group (P〈0.05); combined de- tection of three markers of neonatal asphyxia diagnosis sensitivity, specificity, complicated with myocardial injury were higher than that of single detection (P〈 0. 05). Conclusion: Glycogen phosphorylase isoenzyme BB, plasma N terminal brain natriuretic peptide and ischemia modified albumin as markers can be reflected on the myocardial injury in neonatal asphyxia, combined detection of three markers showed significant diagnostic value in the diagnosis of myocardial injury in neonatal asphyxia.
