腹腔镜与开腹左半结肠癌根治术的短期疗效比较

Comparison of short-term efficacy between laparoscopy and open surgery in left colonic resection for colonic cancer

目的:比较腹腔镜与开腹左半结肠癌根治术的短期疗效。方法:选取接受腹腔镜左半结肠癌根治术的11例患者作为腔镜组,另选取同期接受开腹左半结肠癌根治术的18例患者作为开腹组,比较两组短期疗效。结果:腔镜组在术中出血量、肛门排气时间、住院时间、疼痛及切口愈合不良率方面明显优于开腹组,差异有统计学意义(P<0.05),切口满意度、住院费用明显高于开腹组,差异有统计学意义。两组在手术时间、淋巴结清扫数量、肺部感染、腹腔感染、吻合口瘘、术后肠梗阻及非计划再次手术方面差异无统计学意义(P>0.05)。结论:腹腔镜左半结肠癌根治术具有较好的手术安全性及短期疗效。

Objective：To compare the short-term efficacy between laparoscopy and open surgery in left colonic resection for colonic cancer.Methods：Twenty-nine patients who underwent left colonic resection for colonic cancer from Aug.2014 to Jul.2016 were selected as research object.They were divided into laparoscopic surgery group （n=11） and open surgery group （n=18）.The short-term outcomes were compared between two groups.Results：The intraoperative blood loss,time for passage of gas,hospital stay,incision pain and abnormal wound healing rate in the laparoscopic surgery group were much lower than those in open group,and the differences were statistically significant （P〈0.05）.Incision satisfaction rate and hospitalization expenses in the laparoscopic surgery group were much higher than those in the open group,and the differences were statistically significant （P〈0.05）.There was no significant difference between two groups in operating time,number of dissected lymph node,pulmonary infection,abdominal infection,anastomotic fistula,post-operative intestinal obstruction and unplanned reoperation （P〉0.05）.Conclusions：The safety and short-term efficacy is affirmed in laparoscopic left colonic resection for colonic cancer.
