美托洛尔联合胺碘酮治疗慢性心力衰竭伴室性心律失常的疗效及安全性

Efficacy and Safety of Metoprolol Combined with Amiodarone in Treatment of Chronic Heart Failure with Ventricular Arrhythmia

目的:探讨美托洛尔联合胺碘酮治疗慢性心力衰竭伴室性心律失常的疗效及安全性。方法:选取2013年6月—2016年6月江西省上饶县人民医院收治的慢性心力衰竭伴室性心律失常患者60例作为研究对象,以随机数字表法分为观察组和对照组,每组30例。对照组患者给予胺碘酮治疗,观察组患者在对照组基础上加用美托洛尔治疗。观察两组患者的临床疗效及不良反应发生情况。结果:治疗后,两组患者QT离散度、左心室射血分数均明显高于治疗前,且观察组患者均明显高于对照组,差异均有统计学意义(P<0.05)。观察组患者心功能总有效率为90.00%(27/30),明显高于对照组的63.33%(19/30);观察组患者室性心律失常总有效率为93.33%(28/30),明显高于对照组的73.33%(22/30),差异均有统计学意义(P<0.05)。两组患者不良反应发生率的差异无统计学意义(P>0.05)。结论:美托洛尔联合胺碘酮治疗慢性心力衰竭伴室性心律失常的疗效显著,可有效改善患者的临床症状,无明显不良反应,安全、有效。

OBJECTIVE： To probe into the efficacy and safety of metoprolol combined with amiodarone in treatment of chronic heart failure with ventricular arrhythmia. METHODS： 60 patients with chronic heart failure with ventricular arrhythmia admitted into Shangrao County People＇s Hospital from Jun. 2013 and Jun. 2016 were selected to be divided into observation group and control group via the random number table,with 30 cases in each. The control group were treated with amiodarone,while the observation group additionally received metoprolol based on the control group. The clinical efficacy and incidence of adverse drug reactions of two groups were observed. RESULTS： After treatment,the QT dispersion,left ventricular ejection fraction of two groups were significantly higher than that of before treatment,and the observation group was significantly higher than the control group,with statistically significant difference（ P〈0. 05）. The total effective rate of ventricular function of observation group was 90. 00%（ 27/30）,significantly higher than that that of control group [63. 33%（ 19/30） ]; the total effective rate of ventricular arrhythmias of observation group was 93. 33%（ 28/30）,significantly higher that that of control group [73. 33%（ 22/30） ],with statistically significant difference（ P〈0. 05）. There was no statistical significance between two groups in the incidence of adverse drug reactions（ P〈0. 05）. CONCLUSIONS： The efficacy of metoprolol combined with amiodarone in treatment of chronic heart failure with ventricular arrhythmia is remarkable,which can effectively improve the clinical symptoms of patients with few adverse drug reactions and high safety.
