磁共振成像与高频超声对钙化及非钙化乳腺DCIS诊断分析 被引量:1

摘要 目的探讨动态增强磁共振成像(DCE-MRI)与高频超声(HUS)对钙化及非钙化乳腺导管原位癌诊断的敏感性。方法回顾性分析91例乳腺导管原位癌患者的乳腺X线摄影、动态增强磁共振成像(DCEMRI)与高频超声(HUS)的影像表现,运用Mc Nemar检验分析动态增强磁共振成像和高频超声对乳腺DCIS诊断的敏感性。结果动态增强磁共振成像(DCE-MRI)和高频超声检查(HUS)对乳腺导管原位癌的诊断敏感性分别为91.2%,70.3%.对于51例伴钙化及40例不伴钙化组,动态增强磁共振成像的诊断敏感性分别为90.2%,92.5%,高频超声的诊断敏感性分别为76.5%,62.5%.结论动态增强磁共振成像对乳腺导管原位癌的诊断价值优于高频超声。对于不伴钙化组,动态增强磁共振成像对其诊断较高频超声更有帮助。 Objective To compare the sensitivities of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI) and high frequency ultrasonography(HUS) in pathologically proven calcified and non-calcified DCIS.Methods Ninty-one cases of patients confirmed as breast ductal carcinoma in situ by pathology were retrospectively analyzed.All patients were examined by mammography,high frequency ultrasound and MR imaging.Statistical analysis of the sensitivity of each modality was performed using Mc Nemar's test.Results The total sensitivities of MRI and HUS in the 91 cases were 91.2% and 70.3%,respectively.Of 51 cases with calcifications on mammography,90.2% were detected by MRI and 76.5% by HUS.Of these 40 cases without calcifications on mammography,92.5% were detected by MRI and 62.5% by HUS.Conclusion MRI showed a higher sensitivity for the detection of calcified and non-calcified DCIS and is more helpful than HUS in cases without calcifications on mammography.
