钝针扣眼穿刺技术在维持性血透病人动静脉内瘘中的应用

Application and observation of blunt needle buttonhole technique in arteriovenous fistula puncture

目的探讨钝针扣眼穿刺技术在维持性血透病人动静脉内瘘中的应用效果。方法选取血透病人50例,随机数字表法分为观察组和对照组各25例,观察组给予钝针扣眼穿刺,对照组给予传统方法穿刺,观察6个月后,对两组病人一次穿刺成功例数、拔针按压时间、穿刺疼痛程度及并发症发生情况进行比较。结果观察组穿刺成功率高于对照组,拔针按压时间、穿刺疼痛程度、并发症(穿刺部位渗血、血肿、动脉瘤)均低于对照组(P<0.05)。结论钝针扣眼穿刺技术能提高穿刺成功率,缩短拔针按压时间,降低渗血、血肿、动脉瘤等并发症发生,对延长内瘘使用寿命有重要意义。

Objective To investigate the effect of blunt needle buttonhole puncture technique on arteriovenous fistula puncture in hemo-dialysis patients. Methods Fifty cases of hemodialysis patients were randomly assigned to observation g ro u p （n =2 5 ,blunt needle but-tonhole puncture） and control group （n = 2 5 , ordinary puncture ） . The one-time success puncture ra te , compressing time after pulling out the needle,pain scores and the incidence of complications were compared between two groups after 6 months. Results The obser-vation group had higher one-time success puncture rate, shorter compressing time, lower pain scores and lower incidence of complica-tions （puncture site bleeding,hematoma,aneurysm） than the control group,with statistically significant differences （P 〈 0 . 0 5 ） . Con-clusions The blunt needle buttonhole technique can improve the success rate of p un cture, shorten the compressing time after pulling out the needle, reduce the incidence of complications including bleeding, hematoma and aneurysm, thus playing an important role in pro-longing the life of fistula.
