中医补肾健脾法对HBeAg阳性原发性肝癌伴肝硬化患者免疫功能和生存质量的影响

Effect of tonifying kidney and invigorating spleen of traditional Chinese medicine on immune function and quality of life in patients with HBeAg positive primary liver cancer complicated with liver cirrhosis

目的观察中医补肾健脾法对HBeAg阳性原发性肝癌伴肝硬化患者免疫功能和生存质量的影响。方法将80例HBeAg阳性原发性肝癌伴肝硬化患者随机平行分为治疗组与对照组各40例,2组均给予常规西医抗病毒及对症治疗,治疗组在此基础上加用自拟补肾健脾汤口服,2组疗程均为12周。观察2组临床疗效及治疗前后免疫功能和生存质量情况。结果治疗组临床控制率明显高于对照组(x^2=11.206,P<0.05);治疗组治疗后各项细胞免疫指标(CD3^+、CD4^+、CD8^+、CD4^+/CD8^+)均较治疗前及同期对照组明显改善(P均<0.05),体液免疫指标(IL-2、IL-10、IFN-γ)中除IL-10无明显改善外(P>0.05),其余指标均较治疗前及同期对照组明显改善(P均<0.05);治疗组治疗后QLQ-C30(V3.0)量表评分明显低于治疗前及同期对照组(P均<0.05)。对照组各项指标均无明显改善(P均>0.05)。结论中医补肾健脾法可明显改善HBeAg阳性原发性肝癌伴肝硬化患者免疫功能和生存质量,值得推广应用。

Objective It is to observe the effect of tonifying kidney and invigorating spleen of traditional Chinese medicine on immune function and quality of life in patients with HBeAg positive primary liver cancer complicated with liver cirrhosis. Methods 80 patients with HBeAg positive primar}^ liver cancer complicated with liver cirrhosis were randomly divided into treatment group and control group, each with a total of 40 cases. All patients were treated with conventional western medicine antiviral and symptomatic treatment, on this basis, the patients of the treatment group were treated with tonifying kidney and invigorating spleen decoction, the two groups were treated for 12 weeks. The clinical efficacy and immune function and quality of life were observed before and after treatment in the 2 groups. Results The clinical control rate of the treatment group was significantly higher than that of the control group （P 〈0. 05） . After treatment, the cellular immune indexes （ CD3 ＋ , CD4 ＋ , CD8 ＋ , CD4 ＋ / CD8 ＋ ） of the treatment group were significantly improved than before and the control group （all P 〈 0. 05 ）; the humoral immune index IL-2, IFN-a were significantly improved than l3efore and the control group （all P 〈 0. 05 ） ; the QLQ-C30 （ V3. 0） score of the treatment group was significantly lower than before and the control group （all P 〈0. 05） . There was no significantly improvement in all the al3〇 ve indexes in the control group （P 〉0. 05） . Conclusion Tonifying kidney and invigorating spleen methods of traditional Chinese medicine can significantly improve the immune function and quality of life in patients with HBeAg positive primary liver cancer complicated with liver cirrhosis, it is worthy of popularization and applica-tion.
