PICCO监测在颅高压病人管理中的应用

摘要 目的分析在重度颅脑损伤治疗中,脉搏指示连续心输出量(PICCO)血流动力学监测的作用。方法选取我院2014年12月至2015年12月收治重度颅脑损伤的50例患者,随机分为两组,对照组和研究组,对照组为普通监测组,研究组采用PICCO监测配合无创颅内压监测组。对两组血流动力学指标、临床治疗效果和住院时间进行监测和观察,对所得数据进行统计学分析。结果两组患者的血流动力学指标、临床治疗效果和住院时间,研究组各项均优于对照组。结论 PICCO可以准确监测患者的血流动力学状态,结合无创颅内压监测,对重度颅脑损伤、颅高压病人救治具有较好的指导作用,且操作简单,可靠性、安全性高,可以明显改善患者早中期预后。 Objective To analyse the role of the pulse index continuous cardiac output (PICCO) hemody- namic monitoring in the treatment of the severe craniocerebral injury. Methods Selected 50 cases of severe cran- iocerebral injury patients treated in December 2014 - December 2015, randomly divided into two groups, control group and research group, the control group under ordinary monitoring, the research group under PICCO monitor and intracranial pressure monitoring with noninvasive procedure. To monitoring and observing the hemodynamic parameters, the therapeutic effects and the length of admission in the two groups, collected the data for statistical analysis. Result The research group are better than the control group in the hemodynamic indexes, the thera- peutic effects and length of hospital stay. Conclusion PICCO can more accurately monitor the patient's hemo- dynamic status, combined with noninvasive intracranial pressure monitoring, offer good guidance function on treatment of patients with severe eranioeerebral injury and high cranial pressure . with it ' s simple operation, high reliability and security, PICCO can significantly improve the prognosis of patients in early and middle stage.
