鼻内镜下累及上颌窦或鼻底的上颌骨囊肿开窗切除术 被引量:7

Endoscopic resection of maxillary cyst involving the maxillary sinus or nasal floor under nasal endoscope

摘要 目的探讨采用低温等离子刀行鼻内镜下上颌骨囊肿经下鼻道或鼻腔底开窗切除术的有效性和可行性。方法对23例累及上颌窦或鼻腔底的上颌骨囊肿,术前行鼻窦、上颌骨CT扫描三维重建,了解囊肿与上颌窦、鼻底的关系,采用低温等离子刀鼻内镜引导下经下鼻道或鼻腔底开窗切除术,术后复查和随访。结果所有患者术后未出现明显面部软组织肿胀及面部、牙龈麻木牙齿酸痛、牙龈瘘及咀嚼功能下降等,囊肿术腔黏膜于术后8周左右上皮化。随访6~18个月,囊肿无复发。结论采用低温等离子刀鼻内镜下经下鼻道或鼻腔底开窗切除累及上颌窦或鼻底的上颌骨囊肿,较传统的术式具有视野清晰、微创、术后恢复快,安全有效等优点。 Objective To investigate the effective- ness and feasibility of endoscopic sinus surgery for nasal or nasal floor tinder nasal endoscope. Methods On 23 cases involving the maxillary sinus and nasal cavity bottom jaw cyst, preoperative sinus, maxillary CT scanning 3D recon- struction, understanding cyst and maxillary sinus, nasal floor relationship, using low-temperature plasma ablation of nasal endoscopy under the guidance of the nose or nasal bottom win- dow resection, postoperative review and follow-up. Results There were no obvious facial soft tissue swelling and fa- cial, gingival numbness, pain, gingival fistula and chewing function of all patients. The results showed that the cysts were in 8 weeks after operation. Follow up for 6 to 18 months, no recurrence of cysts. Conclusion The low temperature radiofrequency under nasal endoscope the nose or nasal bottom window to excise involved maxillary sinus or nasal base of cyst of maxilla. Compared with the traditional operation has clear vision, operation is minimally invasive, quick recovery, safe and effective advantages.
