丙氨酰谷氨酰胺不合理使用引发的不良反应情况分析

目的分析丙氨酰谷氨酰胺不合理使用引发的不良反应情况,提高临床用药合理性。方法选取重庆市职业病防治院2014年2月收治的静脉滴注丙氨酰谷氨酰胺患者1245例,分析药物不良反应发生情况,并根据患者用药指征、剂量、时间及药物配比分析不合理使用情况。结果 1245例患者中出现不良反应124例(9.96%)。124例患者中不完全符合用药指征占18.55%,与5%葡萄糖溶液500 ml、0.9%氯化钠溶液500 ml配伍不合理占33.90%,与复方氨基酸500 ml配伍不合理占47.60%,与>1000 ml含脂肪乳、氨基酸、电解质、微量元素的溶液配伍不合理占18.50%。使用时间不合理主要集中在2~10 d。结论药物配比浓度及成分、超范围使用、禁忌证使用是丙氨酰谷氨酰胺不合理使用引发的不良反应的主要原因。

Objective To analysize the adverse events caused by alanylglutamine c unreasonable used, and improve the rationality of clinical using. Methods A total of 1245 cases of patients who were treated with alanylglutamine by intravenous drip were selected in February 2014 in Disease Control and Prevention in Chongqing Municipality, analysized the adverse events and the unreasonable using of medication indications, doses, time and drug dispensing ratio. Results In 1245 cases of pa- tients, adverse reaction situation was 124 cases （9.96%）. In 124 cases of pattens with adverse reaction, incomplete compli- ance with indications for medication was 18.55%, incompatibility unreasonable with 5% glucose solution 500 ml, O. 9% sodi- um chloride solution 500 ml accounted for 33. 90%, and incompatibility unreasonable with compound amino acid 500 ml accounted for 47.60%, incompatibility unreasonable with 〉 1 000 ml solution wbich containing fat milk, amino acids, electrolytes, trace elements accounted for 18.50%. Using time major concentrated on 2 - 10 d. Conclusion Proportion concentration and composition, use overrun, contraindication using are the main cause of adverse events by alanylglutamine unreasonable used.
