摘要
目的分析糖尿病肾病(DN)患者血清降钙素原(PCT)水平与肾功能指标的相关性。方法选取2013年3月-2014年5月住院治疗的糖尿病肾病患者60例作为糖尿病肾病组,同时选取糖尿病非肾病患者60例作为糖尿病组,另选择同期健康体检者60例为健康对照组。检测所有研究对象的PCT水平,并检测相关肾功能相关指标,分析DN患者血清PCT与其他检测指标间的关系。结果糖尿病肾病组患者PCT、P水平明显高于糖尿病组和健康对照组,差异有统计学意义(P<0.05),糖尿病组与健康对照组PCT、P水平比较无显著性差异(P>0.05)。糖尿病肾病组血Cr高于糖尿病组及健康对照组,Ccr低于糖尿病组及健康对照组,且糖尿病组Cr高于健康对照组,Ccr低于健康对照组,差异均有统计学意义(P<0.05)。3组Ca水平比较差异无统计学意义(P>0.05)。Pearson相关性结果发现,PCT水平与血清Cr呈正相关(r=0.457,P<0.05),与Ccr呈负相关(r=-0.405,P<0.05)。糖尿病肾病组患者血清α1-微球蛋白、C-反应蛋白(CRP)、HbA1c、尿Cr的含量明显高于健康对照组,且糖尿病肾病组α1-微球蛋白、CRP及尿Cr均高于糖尿病组,糖尿病组HbA1c高于对照组,差异均有统计学意义(P<0.05)。Pearson相关性结果发现,PCT水平与CRP(r=0.201,P<0.05)及尿Cr水平(r=0.346,P<0.05)呈正相关。预后良好与PCT上升呈负相关(r=-0.345,P<0.05),与PCT下降呈正相关(r=0.312,P<0.05)。肾性高血压、肾衰竭发生与PCT水平呈正相关,与其他并发症无相关性。结论无明显感染迹象的糖尿病肾病患者血清PCT水平增高,这可能与微小炎症灶的存在及肾脏功能受损相关。
Objective To investigate the correlation between serum procalcitonin(PCT) level and renal function in patients with diabetic nephropathy(DN).Methods 60 cases of patients with diabetic nephropathy were selected from March 2013 to May 2014 as diabetic diabetic nephropathy group.60 patients with diabetic non-nephropathy were selected as diabetes group,and 60 healthy subjects were selected as healthy control group.The levels of PCT in all subjects were measured and the related renal function was measured,the relationship between serum PCT and other test indexes was analyzed.Results The level of PCT and P in diabetic nephropathy group were significantly higher than that in diabetic group and healthy control group(P〈0.05),there were no significant difference in PCT and P levels between diabetic group and healthy control group(P〉0.05).The Cr of diabetic nephropathy group was higher than that of diabetes mellitus group and healthy control group,The Ccr of diabetic nephropathy group was lower than that of diabetes mellitus group and healthy control group,and Cr in diabetic group was higher than that in healthy control group,and Ccr was lower than that of healthy control group,the difference were statistically significant(P〈0.05).There was no significant difference in the Ca level between the three groups(P〉0.05).Pearson correlation results showed that PCT level was positively correlated with serum Cr(r=0.457,P〈0.05),and negatively correlated with Ccr(r=-0.405,P〈0.05).The levels of serum α1-microglobulin,C-reactive protein(CRP),HbA1c and urinary Cr in diabetic nephropathy group were significantly higher than those in healthy control group,and diabetic nephropathy group α1-microglobulin,CRP and urine Cr were higher than the diabetic group,diabetic group HbA1c higher than the control group,the difference were statistically significant(P〈0.05).Pearson correlation findings showed that PCT levels were positively correlated with CRP(r=0.201,P〈0.05),and urinary Cr levels(r=0.346,P〈0.05).Pearson correlation results found that,The prognosis was negatively correlated with the increase of PCT(r=-0.345,P〈0.05),the prognosis was positively correlated with the decrease of PCT(r=0.312,P〈0.05).Renal hypertension,renal failure occurred with the PCT level was positively correlated,no correlation with other complications.Conclusion Compared with healthy volunteers,no obvious signs of infection DN elevated serum PCT levels,which may be impaired kidney function and existence and small foci of inflammation-related.
