ICF-CY评估脑瘫患儿身体结构与功能、活动和参与的研究

Application of ICF-CY in evaluating body structure,function,activity and participation of cerebral palsy children

目的运用《国际功能、残疾与健康分类(儿童与青少年版)》(ICF-CY)评估脑瘫患儿身体结构与功能、活动和参与的主要功能障碍特点,为ICF-CY应用于脑瘫患儿功能障碍评估和制定康复计划提供依据。方法采用ICF-CY功能检查表对参加康复训练和治疗的43例脑瘫患儿进行身体结构与功能、活动和参与进行问卷调查和计分评估,并比较康复治疗前后功能障碍的改善情况。结果脑瘫患儿在感觉和疼痛、学习和应用知识、交流、活动、主要生活领域方面均表现为中度障碍,在精神状态、神经肌肉骨骼和运动功能、一般任务和要求等方面均表现为重度障碍。康复治疗对脑瘫患儿功能改善有较好的疗效,表现为ICF-CY评分≥2的中重度障碍者比例均较治疗前显著降低(P<0.05)。结论 ICF-CY可有效发现脑瘫患儿的主要功能障碍及特点,为制定康复计划提供参考和依据,同时也可检验和评估康复计划的治疗效果,实施针对性的康复治疗计划可不同程度地改善脑瘫患儿功能障碍。

Objective This study employed the international classification of functioning,disability and health,children and youth version（ICF-CY） to evaluate body structure,function,activity and participation of cerebral palsy children,in an attempt to provide evidences for evaluating functional disability and rehabilitation plan.Methods ICF-CY functional scale was employed to measure body structure ＆amp; function,activity and participation in a total of 43 cerebral palsy children in the form of questionnaire and scoring.The improvement of functional disorder before and after rehabilitation was analyzed.Results Cerebral palsy children presented as moderate disorder in sensation and pain,learning and applying knowledge,communication,activity and major life fields,and showed severe dysfunction in mental status,neuromuscular-skeletal function,motor function,general task and requirement.Rehabilitation had satisfactory efficacy improvement on functions of cerebral palsy children,as shown by significantly decreased ratio of moderate to severe dysfunctional patients（P〈0.05）.Conclusion ICF-CY can effectively discover major functional disorder and features of cerebral palsy children,and provide references for rehabilitation plan,in addition to test and evaluate the treatment efficacy.Such individualized rehabilitation thus can improve functional disorder in cerebral palsy patients.
