摘要
目的:探讨超声检查在乳腺导管原位癌(DCIS)诊断中的应用价值。方法:l回顾性分析137例乳腺超声检查为BI-RADS4级患者的声像图特征,其中DCIS组55例(40.10%55/137),良性乳腺肿瘤组41例(29.95%41/137),乳腺浸润性癌组41例(29.95%41/137),所有病例均经超声引导下乳腺穿刺活检或手术病理证实,详细记录肿瘤形态、边界、内部回声及CDFI血流信号等各种征象,对三种类型的乳腺肿瘤超声表现进行对比分析。结果:DCIS与良性乳腺肿瘤超声图像表现在肿瘤内部回声、微钙化有统计学差异(P<0.05),DCIS与乳腺浸润性癌的超声图像在肿瘤形态、边界、导管扩张、后方回声衰减有显著差异(P<0.01)。通过CDFI观察,DCIS与良性乳腺肿瘤在动脉RI方面有统计学差异,DCIS与恶性乳腺肿瘤在血流信号(Alder分级)方面有统计学差异(P<0.01)。结论 DCIS声像图表现具有一定的特征性,病灶内微钙化、导管扩张、RI指数高等特征有一定的参考价值。
Purpose: Discuss the application value of ultrasonic using in ductal carcinoma in situ (DCIS). Methode: retrospective analysis ultrasonographic features of 137 cases of BI-RADS 4 patients. Among which, DCISs are 55 cases (40. 10% 55/137), benign mammary tumors are 41 cases (29. 95% 41/137), Mammary invasive carcinomas are41 cases (29. 95% 41/137). All the cases have been verified by Ultrasound guided breast biopsy or surgical pathology. Record in detail have been made about all kinds of signs of tumor morphology, border, internal echo and of CDFI signal, and contrastiveanalysis have been made about the sonographic findings of breast tumor. Results: The statistiesdifference ( P 〈 0. 05 ) of tumor internal echo, microcalcification, responding to DCIS and Mammary invasive carcinoma, and the statisticsdifference (P 〈 0. 01 ) of tumor shape, boundary, duct eetasia, rear echo attenuation shown in the' ultrasound images of the gland invasive carcinoma. With CDFI observation, DCIS and benign mammary gland tumor have statistic difference in artery RI. DCIS and malignant breast tumor have statistic difference ( P 〈 0.01 ) in Blood flow signals ( Alder hierarchical). Conclusion: DCIS ultrasonogram have its own characteristic features. The features of lesions in microcalcification, duct ectasia and high resistance index have a certain reference value.
