消痤汤内服及古方颠倒散外敷治疗痤疮疗效观察

Oral Taking Xiaocuo Tang Combined with External Application with Ancient Formula Diandao San for Acne

目的:观察消痤汤内服及古方颠倒散外敷治疗痤疮的临床疗效。方法:将70例痤疮患者随机分成2组,治疗组40例予消痤汤内服及颠倒散外敷,对照组30例予维胺酯胶囊内服及复方氯霉素醇溶液外搽,治疗1月后观察2组临床疗效。应用放射免疫法测定70例痤疮患者治疗前的性激素水平。结果:治疗前性激素水平的测定结果显示,70例痤疮患者中18例孕酮增高,占25.71%,10例人促卵泡生成素偏低,占14.29%,7例泌乳素增高,占10.00%,余皆正常。临床疗效总有效率治疗组77.50%,对照组50.00%,2组临床疗效比较,差异有统计学意义(P<0.05)。辨证分型肺经风热、肠胃湿热临床疗效分别与痰湿瘀滞比较,差异均有统计学意义(P<0.05),辨证分型肺经风热、肠胃湿热临床疗效比较,差异无统计学意义(P>0.05)。不良反应情况示治疗组有5例患者外敷颠倒散后皮肤轻微瘙痒及轻微发红,于第2周治疗时皮肤瘙痒及发红均消失,余未见异常。结论:消痤汤内服及颠倒散外敷治疗痤疮疗效确切,且用药安全;孕酮增高与痤疮发病可能存在一定联系。

Objective：To observe the clinical effect of oral taking xiaocuo tang combined with external application with ancient formula Diandao san for acne. Methods：Selected 70 acne patients and divided them into two groups. The 40 cases of the treatment group were given oral taking xiaocuo tang combined with external application with Diandao san, and the 30 cases of the control group were given oral viaminate capsules and liniment compound chloramphenicol alcoholic solution,and observed clinical effect of the two groups after one month. sex hormone level was determined by radioimmcuity before treatment. Results：Testing result of sex hormone level before treatment suggested that progesterone of 18 acne patients increased,and the proportion was 25.71%;follicle stimulating hormome（FSH） of 10 patients were on the low side,and the proportion was 14.29%;prolactin of 7 patients increased,and the proportion was 10.00%,others were normal. The total effective rate was 77.50% in the treatment group,and was 50.00% in the control group,the difference being significant（P〈0.05）. Compared clinical effect on wind and heat in the lung meridian type and damp-heat in intestine and stomach type with that on phlegm-dampness stasis type respectively,the difference being significant（P〈0.05）. Compared clinical effect on wind and heat in the lung meridian type with that on damp-heat in intestine and stomach type,the difference was not significant（P〈0.05）. The adverse reaction that appeared suggesting there were slight itching skin and slight redness in 5 patients of the treatment group after using Diandao san. But the slight itching skin and slight redness disappeared in the second week of treatment. No other abnormality was found. Conclusion：Oral taking Xiaocuo tang combined with external application with Diandao san has certain efficacy and it is safe in treating acne; the increase of progesterone may be correlated with the incidence of acne.
