原发性骨质疏松与老年退性心脏瓣膜病的相关性分析

Primary osteoporosis and senile degeneration of valvular heart disease correlation analysis

目的探讨老年退性心脏瓣膜病(senile degenerated heart valvular disease,SDHVD)与骨质疏松(osteoporosis,OP)的关系。方法连续收集2015年9月至2016年9月在我院心内科住院的患者,年龄≥60岁,根据纳入标准,随机抽取151例,两组全做超声心动图,根据老年退性心脏瓣膜病诊断标准,分为老年退性心脏瓣膜病组(SDHVD组)89例,非老年退性心脏瓣膜病组62例作为对照组。比较2组各项生化指标:血钙(Ca^(2+))、25-羟基维生素D3[25(OH)VD3]、甲状旁腺激素(PTH)、骨碱性磷酸酶(B-ALP)、同型半胱氨酸(Hcy);同时对所有研究对象均行腰椎(L1、L2、L3、L4)和股骨近端(Neck、Troch、Ward's)进行双能X线骨密度测定。结果 (1)SDHVD组和对照组组各项生化指标比较,SDHVD组25(OH)VD3、B-ALP、PTH、Hcy值有显著性差异(P<0.05)。(2)SDHVD组和非SDHVD组腰椎、股骨颈骨密度值比较。SCVD组L1、L2、L4、Neck、Troch、Ward's与非SDHVD组对比骨密度有显著性差异(P<0.05)。结论老年退性心脏瓣膜病与骨质疏松之间存在一定的相关性。

Objective To investigate the relationship between senile degenerated hea-rt valvular disease（SDHVD） and osteoporosis（OP）. Methods Eighty-nine patients with recurrent heart disease（SDHVD） and 52 patients with non-aged heart disease（non-SDHVD） were randomly selected according to the inclusion criteria（P〈0.05）. The levels of serum calcium（Ca^2＋）,25-hydroxyvitamin D3 [25（OH） D3],parathyroid hormone（PTH）,bone alkaline phosphatase（B-ALP）,At the same time,dual-energy X-ray absorptiometry was performed on lumbar vertebrae（L1,L2,L3,L4） and proximal femur（Neck,Troch,Ward＇s） for all subjects. Results （1）There were significant differences in the levels of 25（OH） D3,B-ALP,PTH and Hcy in SDHVD group and SDHVD group（P〈0.05）. （2）Comparison of bone mineral density of proximal lumbar vertebrae and femoral neck in SDHVD group and non-SDHVD group. There were significant differences in BMD between SCVD group L1,L2,L4,Neck,Troch,Ward＇s and non-SDHVD groups（P〈0.05）. Conclusion There is a certain correlation between senile heart disease and osteoporosis.
