探究性眼球轨迹运动对轻度认知功能障碍和早期阿尔茨海默病的诊断价值

Value of exploratory eye movement examination in the diagnosis of mild cognitive impairment and early-stage Alzheimer disease

目的:评估探究性眼球轨迹运动(眼动)检查对轻度认知功能障碍和早期阿尔茨海默病的诊断价值。方法:收集轻度认知功能障碍患者(MCI组)、早期阿尔茨海默病患者(早期AD组)、健康人(对照组)各30例,采用蒙特利尔认知评估量表(Mo CA)对全部受试者进行测评;进行眼动检查,测定眼球凝视点数、反应性探索分(RSS)、判别分析值(D值);将MCI组、早期AD组的凝视点数、RSS、D值与Mo CA分值行相关性分析。结果:3组间凝视点数、RSS、D值间差异均有统计学意义(P<0.01);MCI组、早期AD组的Mo CA分值与凝视点数、RSS均呈正相关(均P<0.01),与D值均呈负相关(均P<0.01)。结论:眼动检查可以用来识别轻度认知功能障碍和早期阿尔茨海默病,眼动检查的测量指标可以反映认知功能障碍的严重程度。

Objective： To study the value of exploratory eye movement examination in the diagnosis of mild cognitive impairment or early-stage Alzheimer disease （early-stage AD）. Methods： The subjects were divided into three groups： individuals with mild cognitive impairment （MCI） , patients with early- stage AD, and normal controls, with 30 cases in each group. All the subjects were evaluated with the Montreal Cognitive Assessment （MoCA） and underwent exploratory eye movement examination. The measures of exploratory eye movements include number of eye fixation （NEF） , responsive search score （RSS） and discriminant analysis value（ D value）. The measures of exploratory eye movements were com- pared among three groups, and the associations between MoCA scores and the eye movement measures were examined. Results： There were statistically significant differences in NEF, RSS, and D value among the three groups（P 〈0.01 ）. MoCA scores were positively correlated with NEF and RSS（P 〈0.01 ） and were negatively correlated with D value （ P 〈 0.01 ） in MCI group and early-stage AD group. Conclu- sion ： Exploratory eye movement examination may be used in distinguishing-individuals with MCI or early- stage AD, and the measures of exploratory eye movement may reflect the severity of cognitive impairment.
