摘要
目的探讨2型糖尿病(T2DM)患者血清同型半胱氨酸与胰岛β细胞功能的关系。方法选取2014年1月~2016年9月我院内分泌科收治的118例T2DM患者,根据血清同型半胱氨酸水平分为T2DM合并高同型半胱氨酸组和T2DM合并正常同型半胱氨酸组,两组均行口服葡萄耐量试验及胰岛素释放试验,比较两组临床特征(性别、年龄、病程、BMI、FBG、HbA1c、FINS、血清同型半胱氨酸)的差异,采用稳态模型评估基础胰岛β细胞功能并比较两组的差异。结果两组的性别、年龄、病程、BMI以及FBG、FINS、HbA1c、HOMA-βF、AUCI30-120/AUCG30-120比较,差异无统计学意义(P>0.05)。T2DM合并高同型半胱氨酸组的ΔI30/ΔG30低于T2DM合并正常同型半胱氨酸组,差异有统计学意义(P<0.05)。血清同型半胱氨酸水平与年龄成正相关(r=0.229,P=0.006)。结论 T2DM患者合并高血清同型半胱氨酸者可能与其年龄有关,且早相胰岛素分泌受损加重。
Objective To explore the relationship between serum homocysteine and islet beta cell function in patients with type 2 diabetes mellitus(T2DM).Methods 118 patients with T2 DM in our department of Endocrinology from January 2014 to September 2016 were selected and divided into the T2 DM with high serum homocysteine level group and the T2 DM with normal serum homocysteine level group according to the level of serum homocysteine,oral glucose tolerance tests(OGTT) and insulin secretion tests were performed in the two groups.The clinical features of gender,age,duration,BMI,FBG,HbA1 c,serum homocysteine were compared between the two groups,the steady-state model were used to evaluated the function index of islet beta cell,and which were compared between the two groups.Results There were no significantly differences in sex,age,BMI,duration and the level of FBG,FINS,HbA1 c,HOMA-β,AUCI30-120/AUCG30-120 between the two groups(P〉0.05).The ΔI30/ΔG30in the T2 DM with high serum homocysteine level group was lower than that in the T2 DM with normal serum homocysteine level group,with significant difference(P〈0.05).There were positive correlation between homocysteine and age(r =0.229,P =0.006).Conclusion T2 DM with high serum homocysteine level may be related to age and the early phase insulin secrction were decreased more severly.
