我院2016年门诊糖皮质激素药物不合理使用调查

Investigation on the irrational use of glucocorticoid drugs in outpatients of our hospital in 2016

目的了解我院2016年门诊糖皮质激素药物的使用情况,保障门诊患者的用药安全性和有效性。方法随机抽取2016年1~12月门诊糖皮质激素处方1200张,对所有抽得的处方进行点评,统计糖皮质激素药物的种类、临床诊断等信息,并对不合理处方进行分析。结果 1200张糖皮质激素药物处方中共使用7种糖皮质激素药物,其中口服药1种,注射剂3种,吸入或喷雾剂3种,分别为醋酸泼尼松片、注射用地塞米松磷酸钠、注射用甲泼尼龙琥珀酸钠、氢化泼尼松注射液、布地奈德混悬液、丙酸氟替卡松鼻喷雾剂、沙美特罗替卡松粉吸入剂。经过处方点评,处方中有1064张合理处方,处方合理率为88.67%;不合理处方136张,其中超适应证用药114张(9.50%),给药途径不适宜22张(1.83%)。结论我院2016年门诊糖皮质激素药物的使用情况基本合理,但仍存在滥用的情况,糖皮质激素处方专项点评工作仍需进一步加强。

Objective To investigate the use of glucocorticoid drugs in outpatient department of our hospital in 2016,and to ensure the safety and effectiveness of drug use.Methods 1200 outpatient glucocorticoid prescriptions were randomly selected from January to December in 2016,all drawing prescriptions were reviewed,types and statistics of glucocorticoid drugs in clinical diagnosis and other information were counted,and the unreasonable prescription were analyzed.Results7 kinds of glucocorticoid drugs were useded in the 1200 glucocorticoid drug prescriptions,including 1 kind of oral medicine,3 kinds of injection drugs,3 kinds of inhalation or spray.They were Prednisone Acetate Tablets,Dexamethasone Sodium Phosphate for Injection,Methylprednisolone Sodium Succinate for Injection,Hydroprednisone Injection,Fluticasone Propionate Nasal Spray,Budesonide Suspension,Salmeterol Xinafoate and Fluticasone Propionate Powder for Inhalation.After prescription comment,1064 reasonable prescriptions were founded in all prescriptions,the rate was88.67%.136 irrational prescriptions were founded,and super indications medication were used in 114 irrational prescriptions,accounting for 9.50%;route of administration were not suitable in the other 22 irrational prescriptions,accounting for 1.83%.Conclusion The use of glucocorticoid drugs in outpatient department of our hospital in 2016 is basically reasonable,but there are still abuse cases.The special review of glucocorticoid prescription needs to be further strengthened.
