妊娠期糖尿病孕妇血浆Lp-PLA2水平及其对妊娠结局的影响

Levels of Plasma Lp-PLA2 in Women with Gestational Diabetes Mellitus and Its Influence on Pregnancy Outcomes

目的研究妊娠期糖尿病孕妇脂蛋白相关磷脂酶A2(Lp-PLA2)水平及其对妊娠结局的影响。方法选取2013年1月—2016年2月在我院分娩的80例妊娠期糖尿病孕妇为研究对象,根据血糖控制情况将孕妇分为:血糖控制不良组(40例),血糖控制良好组(40例),另选取同时段在我院分娩的正常孕妇(40例)作为对照组,观察3组孕妇LpPLA2表达水平及妊娠结局差异。结果血糖控制不良组血浆Lp-PLA2表达水平明显高于血糖控制良好组与对照组(P<0.05),血糖控制良好组血浆Lp-PLA2表达水平与对照组比较差异无统计学意义(P>0.05);3组孕妇胎儿宫内生长受限比较差异无统计学意义(P>0.05),血糖控制不良组在胎膜早破、巨大儿、早产儿中发生率明显高于血糖控制良好组与对照组(P<0.05),血糖控制良好组妊娠结局与对照组比较差异无统计学意义(P>0.05)。结论在妊娠期糖尿病孕妇中血浆Lp-PLA2水平与妊娠期血糖控制情况密切相关,妊娠期血糖控制不佳会导致不良妊娠结局发生率增加。

Objective To study the level of lipoprotein-related enzyme A2（Lp-PLA2）in women with gestational diabetes mellitus（DM）and its effect on pregnancy outcomes.Methods Eighty pregnant women with abnormal blood glucose treated in our hospital between January 2013 and February 2016 were selected as subjects.According to the effect of blood glucose control,these pregnant women were divided into the poor glycemic control group（40 cases）and good blood glucose control group（40 cases）,with 40 normal pregnant women as the control group.The expression levels of Lp-PLA2 in the three groups and the difference of pregnancy outcomes were observed.Results The level of plasma Lp-PLA2 in the poor glycemic control group was significantly higher than that of good glycemic control group and the control group（P〈0.05）.There was no difference in the expression level of plasma Lp-PLA2 between the good glycemic control group and the control group（P〉0.05）.There was no significant difference in the intrauterine growth restriction between the three groups（P〈0.05）.The incidence of premature rupture of membranes,macrosomia and premature infants in the poor glycemic control group was significantly higher than that of the control group and good glycemic control group（P〉0.05）.There was no significant difference in pregnancy outcomes between the good glycemic control group and the control group（P〉0.05）.Conclusion The level of Lp-PLA2 of gestational diabetes mellitus is closely related to the control of gestational diabetes mellitus,and poor control of gestational diabetes mellitus will increase the incidence of adverse pregnancy outcomes.
