射频消融联合放射性^125I粒子植入对肝癌切除术后患者免疫功能和治疗效果的影响 被引量:2

Effects of Radiofrequency Ablation Combined with 125I Radioactive Particles Implantation on Immune Function and Therapeutic Efficacy of Hepatic Resection Patients

摘要 目的探讨射频消融(RFA)联合放射性^125I粒子植入对肝癌切除术患者免疫功能及治疗效果的影响。方法选择我院2006年4月-2009年7月收治的80例肝癌患者为研究对象,随机分为对照组和观察组,每组40例。对照组给予常规肝癌切除术,观察组在肝癌切除术后,对患者切缘行RFA联合^125I粒子植入,观察两组患者的免疫功能、血清肿瘤标志物和近远期疗效。结果①免疫功能:术后7d、14d、28d,观察组CD^+、CD3^+明显高于对照组,CD8^+明显低于对照组(P〈0.05);②肿瘤标志物:术后7d、14d、28d,观察组血清AFP含量明显低于对照组(P〈0.05);③疗效:观察组缓解率明显高于对照组(P〈0.05),两组控制率比较差异无统计学意义(P〉0.05);术后1、3、5年,观察组生存率明显高于对照组(P〈0.05)。结论RFA联合放射性^125I粒子植入有助于增强患者的免疫功能,降低甲胎蛋白表达量,改善肝癌切除术后患者的预后。 Objective To investigate effects of radiofrequency ablation ( RFA ) combined with 1251 radioactive particles implantation on immune function and therapeutic efficacy of hepatic resection patients. Methods 80 patients with liver cancer from Apr. 2006 to Jul. 2009 were randomly divided into control group and observation group with 40 patients in each group. Routine hepatic resection was used in control group. RAF combined with J25I radioactive particles implantation was used after routine hepatic resection in observation group. Immune function, serum tumor markers ,shortterm and longterm efficacy were observed in the two groups. Results CD4^+ and CD3^+ were significantly higher, CD^8+was lower in observation group than those in control group 7 d, 14 d and 28 d after operation (P 〈 0. 05 ) ; Serum AFP levels were significantly lower in observation group than those in control group 7 d, 14 d and 28 d after operation( P 〈 0.05 ). The remission rate was significantly higher in observation group than that in control group ( P 〈 0. 05 ). There was no significant difference in control rate between the two groups ( P 〉 0. 05 ) ;The survival rates were significantly higher in observation group than those in control group 1,3 and 5 years after operation( P 〈0.05 ). Conclusion RFA combined with J25I radioactive particles implantation contribute to enhance immune function, reduce AFP expression and improve the prognosis of hepatic resection patients.
