阿奇霉素配伍脂溶性维生素对支原体肺炎患儿血清炎症因子、趋化因子及肺功能的影响 被引量:37

摘要 目的:研究阿奇霉素配伍脂溶性维生素对支原体肺炎患儿血清炎症因子、趋化因子及肺功能的影响。方法:收集本院收治的支原体肺炎患儿56例,回顾性分析治疗方法及实验室检查结果,分为接受单纯阿奇霉素治疗的对照组26例、接受脂溶性维生素配伍阿奇霉素治疗的观察组30例。治疗前、治疗1周后,采用酶联免疫吸附法(ELISA)检测血清炎症指标、趋化因子含量;采用儿童肺功能监测仪检测肺功能指标水平。结果:治疗前,两组血清炎症指标、趋化因子含量以及肺功能指标的差异无统计学意义(P>0.05)。治疗后,观察组血清中炎症指标IL-4、IL-13的含量低于对照组,IL-10、IL-12的含量高于对照组(P<0.05);观察组血清中趋化因子MCP-4、MDC、CysLTs的含量低于对照组(P<0.05);观察组肺功能指标V-T、FEV1、PEF、MMEF_(2 5%-7 5%)的水平高于对照组(P<0.05)。结论:脂溶性维生素联合阿奇霉素可降低肺炎支原体感染小儿的全身炎症反应,优化肺功能。 Objective:To study the effect of azithromycin combined with fat-soluble vitamin on serum inflammatory cytokines and chemokines as well as lung function in children with Mycoplasma pneumoniae.Methods:A total of 56 children with mycoplasma pneumonia admitted in our hospital between May 2012 and May 2016 were collected,the treatment methods and laboratory test results were retrospectively analyzed,and then they were divided into the control group(n=26)who received azithromycin treatment alone and the observation group(n=30)who received fat-soluble vitamin combined with azithromycin treatment.Before treatment and after 1week of treatment,enzyme-linked immunosorbent assay(ELISA)was used to detect the serum levels of inflammatory cytokines and chemokines;lung function monitor for children was used to detect lung function index levels.Results:Before treatment,differences in serum inflammation index and chemokine contents as well as lung function indexes were not statistically significant between two groups of patients(P〉0.05).After treatment,serum inflammation indexes IL-4and IL-13 contents of observation group were lower than those of control group while IL-10 and IL-12 contents were higher than those of control group(P〈0.05);serum chemokines MCP-4,MDC and CysLTs contents of observation group were lower than those of control group(P〈0.05);lung function indexes V-T,FEV1,PEF and MMEF_(2 5%-7 5%)levels of observation group were higher than those of control group(P〈0.05).Conclusions:Fat-soluble vitamin combined with azithromycin can reduce the systemic inflammatory response and optimize the lung function in children with mycoplasma pneumoniae infection.
