中药“肾痿复方”对CKD血液透析患者生存质量的影响

Effect of traditional Chinese medicine "Shenwei prescription" on the quality of life of patients with CKD undergoing hemodialysis

目的:探讨中药"肾痿复方"对慢性肾脏病(chronic kidney disease,CKD)维持性血液透析患者生存质量的影响。方法:随机抽取我院透析中心符合纳入标准的CKD患者86例,分为透析联合中药治疗组(A组,32例)及透析对照组(B组,54例)。B组治疗采用维持性血液透析治疗,A组在透析基础上加服中药"肾痿复方"加减方丸剂进行干预,两组临床观察12周。所有患者均于治疗前后检测营养状况(含体重指数、血红蛋白);生存质量主观评分由患者按世卫组织(WHO)生存质量测定量表简表进行1~5级自主评分。计量资料采用配对样本t检验,计数资料采用χ2检验。结果:1.营养指标方面,A组治疗后体重指数与B组比较无统计学差异(P>0.05),A组所有患者血红蛋白治疗前后均值差异有统计学意义(P<0.05),B组≥55岁患者治疗前后差异更明显(P<0.01);2.主观评分方面,A、B两组患者在社会关系领域评分差异有统计学意义(P<0.05),其它领域A组相比B组有改善趋势,但差异无统计学意义(P>0.05)。结论:中药"肾痿复方"可以改善长期维持性血液透析患者的生存质量。

Objective： To explore the effect of Shenwei prescription on the quality of life in CKD patients undergoing hemodialysis patients. Methods： 86 CKD patients undergoing maintenance hemodialysis in our dialysis center were randomly divided into 2 groups： dialysis combined with TCM treatment group（group A, 32 cases） and dialysis treatment group（group B, 54 cases）. Group B were treated with basic maintenance hemodialysis therapy compared to group A which were treated with TCM ＂Shenwei prescription＂ in addition to the hemodialysis, and both groups were clinically observed for 12 weeks. Nutritional status of all patients were detected before and after treatment （including body mass index （BMI） and hemoglobin）; The subjective score of life quality was measured by patients according to World Health Organization（WHO） life quality scale ranging from 1~5. The data were analyzed with paired samples t test or ？字2 test accordingly. Results： After treatment, BMI between group A and group B had no statistical difference （P 〉 0.05）. The changes of HGB in group A were statistically significant after treatment（P 〈 0.05）, and in patients ≥55 years old the changes of HGB were much significant in group B after treatment（P 〈 0.01）. There was statistical significance between the two groups in terms of their social life （P 〈 0.05）, and in other areas there was a trend of improvement in group B but it did not reach statistical significance （P 〉 0.05）. Conclusion： “Shenwei prescription” can improve the quality of life in CKD patients undergoing long-term maintenance hemodialysis.
