润脾汤联合沙格列汀治疗脾阴虚证型糖尿病的疗效研究

Clinical efficacy of spleen yin deficiency syndrome type of diabetes patients treated with Spleen Qi Decoction Combined with Saxagliptin

目的研究润脾汤联合沙格列汀治疗脾阴虚证型糖尿病的临床效果。方法选取2013年1月至2015年1月治疗的98例脾阴虚证型糖尿病患者作为研究对象,按照随机数表法分为治疗组和对照组各49例。对照组给予沙格列汀药物治疗,治疗组给予润脾汤联合沙格列汀治疗。监测患者治疗前后空腹血糖、餐后2小时血糖(2 hP G)、糖化血红蛋白(HbA1c)、空腹胰岛素、天门冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、血尿素氮、血肌酐、总胆固醇、甘油三酯、低密度脂蛋白-胆固醇(LDL-C)、高密度脂蛋白-胆固醇(HDL-C)等指标。结果治疗后两组患者血脂、肝功能、肾功能指标较治疗前有一定程度改善,且治疗组患者的LDL-C、ALT、肌酐和血尿素氮水平较对照组降低更明显(P<0.05)。治疗12周后,两组患者的空腹血糖、2 hP G、HbA 1c明显降低,治疗组下降程度优于对照组(P<0.05);空腹胰岛素明显升高,治疗组上升程度优于对照组(P<0.05)。治疗组的总有效率为87.8%,高于对照组的总有效率71.4%(P<0.05)。两组患者均有1例出现低血糖反应,对照组出现1例腹胀、恶心。结论联合使用润脾汤及沙格列汀对脾阴虚证型糖尿病患者有良好的临床疗效,可以有效降低患者的血糖,控制患者血脂,缓解患者的不适症状,安全性较高,值得临床推广。

Objective To study the clinical effect of Spleen Yin decoction combined with Saxagliptin in the treatment of type 2 diabetic patients with spleen yin deficiency syndrome.Methods The 98 cases with spleen yin deficiency syndrome type of diabetes treated in our hospital were retrospectively analyzed from January 2013 to January 2015.They were randomly divided into treatment group and control group,49 cases in each group.The control group was given the Saxagliptin treatment, the treatment group was given moisten spleen decoction combined with Saxagliptin therapy.2hPG, HbA1c, fasting blood glucose, fasting insulin, aspartate aminotransferase, alanine aminotransferase, blood urea nitrogen, serum creatinine, total cholesterol, triglyceride, low density lipoprotein cholesterol, high density lipoprotein cholesterol and other indicators were monitored before and after treatment.Results After treatment, the blood lipid, liver function, renal function indicators in the two groups had a certain improvement than before treatment.The levels of LDL-C, ALT, creatinine and blood urea nitrogen in the treatment group were significantly lower than those in the control group （P〈0.05）.After 12 weeks of treatment, the fasting blood glucose, 2 hPG and HbA1c in the two groups decreased significantly, the treatment group decreased more rapidly than the control group （P〈0.05）;fasting insulin increased more significantly in the treatment group than that of control group （P〈0.05）.After drug treatment, the total effective rate of the treatment group was 87.8%, higher than that of the control group 71.4% （P〈0.05）.1 case of hypoglycemia reaction occurred in the two groups.There was 1 case of abdominal distention, nausea in the control group.Conclusion Combined use of Yun PI Decoction and Saxagliptin of spleen yin deficiency syndrome in patients with type 2 diabetes mellitus has good clinical curative effect, can effectively reduce the patients＇ blood sugar, control blood lipids in patients, remission of symptoms, higher security, it is worth clinical promotion.
