血液透析及血液透析滤过对慢性肾衰竭患者贫血的影响

Effect of Haemodialysis and Hemodiafiltration on the Anemia of Patients with Chronic Renal Failure

目的在慢性肾衰竭合并贫血患者治疗过程中应用血液透析以及血液透析滤过,探析对患者贫血的影响。方法利用自愿参与原则自该院2015年8月—2016年8月期间所收治的慢性肾衰竭合并贫血患者中方便选取56例参与该次探究,给予所有患者促红细胞生成素、叶酸以及铁剂治疗。根据透析方式不同将所有患者分为研究组和参照组,每组患者例数均为28例。在参照组患者治疗过程中应用血液透析,在研究组患者治疗过程中应用血液透析滤过,对比观察2组患者治疗前后的Hb(血红蛋白)、HCT(红细胞压积)、Sf(血清铁蛋白)、hs-CRP(超敏C反应蛋白改善程度)。结果研究组患者治疗前的Hb(62.3±4.0)g/L、HCT(21.3±4.5)%、Sf(68.3±6.2)μg/L、hs-CRP(10.5±3.6)mg/L对比参照组患者差异无统计学意义(P>0.05);研究组患者治疗12周后的Hb(100.3±6.3)g/L、HCT(32.5±2.5)%、Sf(134.2±14.5)μg/L、hs-CRP(8.1±3.1)mg/L显著性优于参照组,差异有统计学意义(P<0.05)。结论在给予慢性肾衰竭合并贫血患者相同贫血药物治疗后再给予患者血液透析滤过可以显著提升临床疗效,且临床疗效明显优于血液透析。

Objective To study the effect of haemodialysis and hemodiafiltration on the Anemia of patients with chronic renal failure. Methods 56 cases of patients with chronic renal failure and anemia admitted and treated in our hospital from August 2015 to August 2016 were conveniently selected and treated with recombinant human erythropoietin, folic acid and iron gent, and all patients were divided into two groups with 28 cases in each according to different haemodialysis methods,the research group applied the hemodiafiltration, while the control group applied the haemodialysis, and the Hb, HCT, Sf and hs-CRP before and after treatment of the two groups were compared. Results There were no obvious differences in the Hb, HCT, Sf and hs-CRP before treatment between the two groups, and the Hb, HCT, Sf and hs-CRP before treatment in the research group were respectively（62.3±4.0）g/L,（21.3±4.5）%,（68.3±6.2）μg/L and（10.5±3.6）mg/L, and the differences were not statistically significant（P〉0.05）, after 12-week treatment, the Hb, HCT, Sf and hs-CRP in the research group were respectively（100.3±6.3）g/L,HCT（32.5±2.5）%,Sf（134.2±14.5）μg/L,hs-CRP（8.1±3.1）mg/L, which were obviously better than those in the control group, and the differences were obvious with statistical significance（P〈0.05）. Conclusion The treatment of the same anemia drugs and hemodiafiltration for patients with chronic renal failure and anemia can obviously improve the clinical curative effect and the clinical curative effect is obviously better than that of haemodialysis.
