三维CT重建股骨头坏死体积、分布、形状的测定及塌陷预测研究

Measurement of Volume, Distribution, Shape Collapse Prediction of Femoral Head Necrosis by 3D-CT Reconstruction

目的探讨三维CT重建股骨头坏死模型,测定坏死病灶的体积、分布、形状,为临床治疗提供参考依据。方法方便选取2013年6月—2015年6月该院收治的53例(87髋)行MRI检查确诊为ARCOⅡ期股骨头坏死患者,所有患者双髋均行三维CT扫描,获得髋关节三维CT图像数据。并采用股骨头坏死测量软件提取股骨头坏死区域,读取股骨头坏死体积、分布,同时提取股骨头坏死的立体形状。结果三维CT重建股骨头坏死模型可见股骨头坏死区域多位于股骨头的外上方,测得整个股骨头的体积为(50 260.3±5 622.5)m3,与实际测量结果差异无统计学意义(P>0.05);计算机测量塌陷的54髋中股骨头坏死病灶占整个股骨头的体积为42.17%,无塌陷的33髋中股骨头坏死病灶占整个股骨头的体积为21.58%。结论三维CT重建股骨头坏死模型,能够清晰、直观的测量病灶的体积、形状、分布,为临床治疗方案的选择提供可靠参考依据。

Objective To study the reconstruction of femoral head necrosis with three-dimensional CT, and to determine the volume, distribution and shape of necrotic lesion, so as to provide reference for clinical treatment. Methods Convenient selection from June 2013 to June 2015, our hospital line 53 cases（87 hips） of MRI diagnosis of ARCO Ⅱ options were osteonecrosis patients, all patients with double hip line of three-dimensional CT scan, hip three-dimensional CT image data obtained. And USES the avascular necrosis of femoral head necrosis area is extracted measurement software, read the volume of femoral head necrosis, distribution, and extract the stereo shape of femoral head necrosis. Results Three-dimensional reconstruction of the femoral head necrosis of the femoral head necrosis model can be seen in the femoral head necrosis more than the top of the outer area, the volume of the femoral head was（50 260.3 ± 5 622.5） m^3, which was not significantly different from the actual measurement（P〉0.05）. The volume of the femoral head necrosis was 42.17% in 54 hips of the 33 femoral head necrosis of femoral head accounted for 21.58% of the total volume of the head. Conclusion Three-dimensional CT reconstruction of the femoral head necrosis model can clearly and intuitively measure the volume, shape and distribution of the lesion, and provide a reliable reference for clinical treatment options.
