姜黄益智胶囊对血管性痴呆痰瘀阻络证患者脑动脉狭窄及认知功能的干预作用

Effects of turmeric and intelligent-improve capsule on cerebral artery stenosis and cognitive function in vascular dementia patients with phlegm-blood stasis syndrome

目的观察姜黄益智胶囊治疗血管性痴呆(VD)的临床疗效。方法将81例VD患者随机分为2组,对照组40例予奥拉西坦胶囊治疗,治疗组41例服用姜黄益智胶囊。2组疗程均6个月。2组治疗前及治疗6个月后均对其进行蒙特利尔认知评估量表(Mo CA)评分、日常生活活动能力量表(ADL)评分、中医证候积分及脑多普勒超声(TCD)、磁共振血管造影(MRA)检查,并统计疗效。结果治疗组有效率87.80%,优于对照组(65.00%,P<0.05)。2组治疗后Mo CA各项积分及总积分较治疗前均升高(P<0.05);治疗组治疗后Mo CA各积分均高于对照组(P<0.05)。2组治疗后ADL、中医证候积分较治疗前均降低(P<0.05);治疗组治疗后ADL、中医证候积分均低于对照组(P<0.05)。2组治疗后基底动脉、颈动脉颅内段均值流速(Vm)均升高,搏动指数(PI)均降低(P<0.05);治疗组治疗后Vm、PI改善情况优于对照组(P<0.05)。2组治疗后血管狭窄程度均改善(P<0.05),治疗组治疗后血管狭窄程度低于对照组(P<0.05)。2组治疗期间均未出现不良反应。结论姜黄益智胶囊能明显改善VD患者认知功能、日常生活能力及脑动脉结构和功能,用药安全。

Objective To observe the clinical effects of turmeric and intelligent - improve capsule on the treatment of vascular dementia （VD）. Methods 81 VD patients were randomly divided into two groups. The con-trol group （n = 40） was treated by Oraxitan capsule, and the treatment group （n =41） was treated by turmeric and intelligent - improve capsule. The treatment course was 6 months in two groups. The scores of montreal cognitive as-sessment scale （MoCA） , daily activity ability scale （ADL） and TCM syndrome, Transcranial Doppler（TCD） and magnetic resonance angiography （MRA） before and after treatment 6 months were measured in two groups , and the curative effects were evaluated. Results The total effective rate in treatment group （87. 80% ） was superior to that in control group （65.0% , P 〈0. 05） . The each score and total scores of MoCA after treatment were increased in two groups （P 〈 0. 05 ）. The each score of MoCA after treatment in treatment group was higher than that in control group （P 〈 0. 05 ）. The scores of ADL and TCM syndrome after treatment were reduced in two groups （ P 〈 0. 05 ） , and which in treatment group were lower than those in control group （P 〈0. 05）. The mean velocites（ Vm） of basilar ar-tery and the intracran - inal of carotid artery after treatment were increased in two groups, and the Pulsation index （PI） was decreased （P 〈0. 05）. The improvement of Vm and PI in treatment group were better than those in control group （P 〈 0. 05 ） . The degrees of vascular stenosis after treatment were improved in two groups （ P 〈 0 . 05 ） , and which in treatment group was lower than that in control group （ P 〈 0. 05 ） . There were no adverse reactions occurred in two groups during treatment. Conclusion Turmeric and intelli-gent -improve capsule can significantly improve the cogni-tive function of VD patients, daily living ability and cerebral artery structure and function, with medication safety.
