彩超引导脐静脉导管定位留置在新生儿脐静脉置管术中的应用

Application of Colour Doppler-Guided Umbilical Venous Catheterization in Neonates

目的探讨彩超引导脐静脉导管定位留置在新生儿脐静脉置管(UVC)术中的应用效果。方法选取2016年1—9月接受治疗、需进行UVC治疗的新生儿50例。按照入院顺序抽签后分为治疗组和对照组,每组25例。对照组常规UVC置管,治疗组彩超引导下UVC置管。比较患儿C-反应蛋白(CRP)和降钙素原(PCT)水平、并发症发生情况、置管成功率和置管留置时间。结果治疗组患儿CRP、PCT水平均低于对照组(P<0.05);治疗组并发症总发生率低于对照组(4.00%比26.00%,P<0.05);治疗组一次性置管成功率高于对照组(100%比76%,P<0.05),而置管留置时间也高于对照组[(11.34±1.22)d比(9.37±1.34)d,P<0.05]。结论彩超引导脐静脉导管定位留置在新生儿UVC效果较好,患儿炎症反应轻,并发症发生率低,一次性成功置管率高,置管留置时间长。

Objective To explore the efficacy of colour Doppler-guided umbilical venous catheterization(UVC)in neonates.Methods Fifty neonates treated between January 2016 and September 2016 were selected in this study.Theses neonates were randomly assigned to receive either conventional UVC(control group,n=25)or colour Doppler-guided UVC(treatment group,n=25).The levels of C reactive protein(CRP)and procalcitonin(PCT),incidence of complications,success rate of catheterization,and catheter indwelling time were compared between the two groups.Results Compared with control group,colour Doppler-guided UVC reduced the levels of CRP and PCT,decreased the incidence of complications(4.00%vs 26.00%),increased the success rate of catheterization on the first attempt(100% vs 76%),and prolonged the catheter indwelling time((11.34±1.22)days vs(9.37±1.34)days)(P<0.05).Conclusion Compared with conventional UVC,colour Doppler-guided UVC can reduce inflammatory response and complications and increase catheterization success rate and catheter indwelling time in neonates.
