早期活动对口腔癌根治性切除加皮瓣移植术后患者康复的影响

Effect of Early Activity on the Recovery of Patients after the Radical Resec-tion of Oral Cancer and Grafting Transposition of Skin Flap

目的分析早期活动对口腔癌根治性切除加皮瓣移植术后患者康复的影响。方法回顾性分析该院2013年6月—2016年6月期间收治的80例口腔癌患者的临床资料,所有患者均行根治性切除加皮瓣移植术治疗;根据双盲法将其分为研究组(40例)与对照组(40例)。对照组40例口腔癌患者给予常规护理,研究组40例口腔癌患者基于常规护理给予早期活动干预;对比两组口腔癌患者的腰背痛发生率、压疮发生率、一次拔尿管成功率、留尿管时间以及血循环危象发生率。结果研究组40例口腔癌患者的一次拔管成功率、血循环危象发生率分别为100%、2.5%,对照组40例口腔癌患者的一次拔管成功率、血循环危象发生率分别为92.5%、5%,两组比较差异无统计学意义(P>0.05)。研究组40例口腔癌患者的腰背痛发生率、压疮发生率以及留尿管时间分别为5%、0%、(40.15±6.31)d,对照组40例口腔癌患者的腰背痛发生率、压疮发生率以及留尿管时间分别为32.5%、17.5%、(52.82±5.47)d,两组差异有统计学意义(P<0.05)。结论早期活动应用于口腔癌根治性切除加皮瓣移植术后患者护理中的效果十分显著,有利于进一步促进患者的快速康复,值得在临床护理工作中推广运用。

Objective To analyze the effect of early activity on the recovery of patients after the radical resection of oral cancer and grafting transposition of skin flap. Methods The clinical data of 80 cases of patients in our hospital from June 2013 to June 2016 were retrospectively analyzed and all patients were treated with radical resection of oral cancer and grafting transposition of skin flap and divided into two groups with 40 cases in each, the control group were given the routine nursing, while the research group were given the routine nursing and early actively intervention, and the incidence rate of lumbodorsal pains, incidence rate of pressure cores, success rate of one-time urinary catheter removal, time for detained catheter and incidence rate of blood circulation crisis were compared between the two groups. Results There were no obvious differences in the success rate of one-time catheter removal and incidence rate of blood circulation crisis between the research group and the control group without the statistical significance（100%,2.5% vs 92.5%,5%）（P〉0.05）, and the differences in the incidence rate of lumbodorsal pains, incidence rate of pressure cores and time for detained catheter between the research group and the control group were statistically significant[5%,0%,（40.15±6.31）d vs 32.5%,17.5%,（52.82±5.47）d]（P〈0.05）. Conclusion The application effect of early activity in the recovery of patients after the radical resection of oral cancer and grafting transposition of skin flap is very obvious, which is conducive to the rapid recovery of patients, and it is worth promotion and application in the clinical nursing.
