聚桂醇联合普萘洛尔治疗婴幼儿血管瘤临床路径探索 被引量:3

摘要 目的:观察聚桂醇局部注射联合普萘洛尔口服治疗婴幼儿血管瘤的临床疗效并探索血管瘤的临床路径。方法:选择我科2013-12~2015-12接受聚桂醇血管瘤体内注射联合普萘洛尔口服治疗的86例婴幼儿血管瘤患者的临床资料进行回顾性分析,探讨疗效,并对血管瘤的路径化诊疗模式进行初步探讨。结果:86例婴幼儿血管瘤患者治愈58例(67.4%),显效14例(16.3%),好转12例(14.0%),无效0例(0.0%),失访2例(2.3%)。经评估、治疗、健康宣教、精确给药、临床监测以及对症处理、对家长进行用药指导、出院指导、院外随访、电话随访及调查显示满意度为98.6%。用药过程中,局部聚桂醇治疗后均出现不同程度肿胀,无需处理,所有患儿3d内肿胀消退。10例患儿注射治疗后出现发热,儿科会诊后对症治疗,8h内热退。46例患儿在服用普萘洛尔1~3h后出现心率减慢(5~12次/min不等),8h后基本恢复服药前水平,未发生严重不良反应。结论:聚桂醇局部注射联合普萘洛尔口服治疗婴幼儿血管瘤是一种安全有效的治疗方法,可在临床进行路径化管理。 Objective: To observe the effect and safety of polidocanol combined with propranolol in the treatment of infantile hemangiomas and explore a clinical pathway.Methods: The clinical date of 86 cases infantile hemangiomas who had accepted polidocanol combined with propranolol for the treatment in our hospital from 2013 December to 2015 December were analyzed retrospectively, clinical pathway of infantile hemangiomas were explored.Results. Fifty -eight cases were cured after treatment, significant effect were observed in 14 cases, 12 cases were effective,2 cases were lost to follow up. Series measures were taken, such as evaluation, therapy,health education, accurate dose and time medicine control, close monitoring, without delay symptomatic treatment, medical guidance to parents and all-time follow-up visit. The degree of satisfaction as high as 98.6%. Swelling were observed in all of the patients after polidocanol intracavitary injection and subsided within 3 days without any intervention. High temperatures were observed in 10 cases after injection and were brought down in 8 hours.The heart rates of 46 cases were slowed down after taking propranolol,recovered to normal level in 8 hours. There was no other serious adverse reactions. Conclusion: Polidocanol combined propranolol is an effective and safe treatment of infantile hemangiomas.It can be popularized as a pathway in clinical practice.
