通窍活络化痰逐瘀汤治疗缺血性脑卒中临床研究

Clinical Research on Cerebral Ischemic Stroke Treated by Orifice-Opening,Collateral-Activating,Phlegm-Resolving and Blood-Stasis-Dispelling Decoction

目的:观察通窍活络化痰逐瘀汤治疗缺血性脑卒中的临床疗效。方法:选取合并高血压、高血脂、高血糖的缺血性脑卒中患者188例,随机分为对照组和观察组各94例,对照组采用奥扎格雷钠、胞磷胆碱以及肌氨肽苷治疗,观察组在对照组治疗的基础上采用通窍活络化痰逐瘀汤治疗。分别于治疗前后抽取两组患者血清行SOD、NO、NOS、MDA检测,并使用神经功能缺损评分对两组患者神经功能进行评价。结果:两组患者治疗前SOD、NO、NOS、MDA水平和神经功能差别不大(P>0.05);观察组治疗后SOD、NO分别为(105.10±6.10)U·m L^(-1)、(72.91±5.38)umol·L^(-1),均明显高于对照组,MDA和神经功能缺损评分分别为(2.19±0.45)μmol·L^(-1)和(11.45±1.27)分,均明显低于对照组,两组比较差异有统计学意义(P<0.05)。结论:通窍活络化痰逐瘀汤可以改善脑细胞代谢,促进神经功能恢复,对于治疗合并高血压、高血脂、高血糖的缺血性脑卒中患者具有积极作用。

Objective： To observe the clinical effects of Orifice-Opening, Collateral-Activating, Phlegm-Resolving and Blood-Sta- sis-Dispelling Decoction on cerebral isehemic, stroke. Methods： Totally 188 patients with cerebral ischemic stroke complicated with hypertension, hyperlipidemia and hyperglycemia were randomized into control group and observation group, with 94 cases in each group. Control group were given treatment of Sodium Ozagrel, Citieoline and Muscular Amino Acids and Nucleosides while observation group were added with Orifice-Opening, Collateral-Activating, Phlegm-Resolving and Blood-Stasis-Dispelling Decoc- tion. The levels of SOl）, NO, NOS and MDA in the serum of both groups were detected. And the neurological function of both groups was evahmted by neurologic imparment scoring method. Results： The levels of SOD, NO, NOS and MDA and neurological function in the two groups had little difference before treatment （ P 〉 0.05）. Afler treatment, the levels of SOD and NO in treat- ment group were respectively （ 105.10 ±6.10） U·mL-1 and （72.91 ± 5.38） U·mL-1 , both higher than those in control group. The scores of MDA and neurologic impairment in observation group were respectively （2. 19 ± 0.45 ）U·mL-1amd （ 11. 45± l. 27）, obviously lower than those in control group; the difference had statistical significance （ P 〈 0.05 ）. Conclu- sion： Orifice-Opening, Collateral-Activating, Phlegm-Resolving anti Bkod-Stasis-Dispelling Decoction can improve the cell me- labolism, promote the neural functional recover3＇ and has positive effects on eerehral ischemic stroke complicated with hyperten- sion, hyperlipidemia and hyperglycemia.
