半乳糖凝集素-3与慢性心力衰竭的相关性研究 被引量:4

摘要 目的 探讨血浆半乳糖凝集素-3(Gal-3)水平对慢性心力衰竭(CHF)的临床价值.方法 选取(纽约心脏协会心功能分级Ⅱ~Ⅳ级)慢性心力衰竭患者120例及对照组40例.酶联免疫吸附法测定血浆Gal-3和NT-proBNP水平.超声心动图检查测定左室舒张末期内径(LVEDD)及左室射血分数(LVEF).受试者工作特征曲线(ROC)分析Gal-3或NT-proBNP水平对CHF的诊断价值.结果 心衰组Gal-3水平显著高于对照组[19.5(10.21~22.59)ng/ml比5.75 (4.66~13.89)ng/ml,P<0.05],且随心功能分级的增加而升高.血浆Gal-3水平与NT-proBNP(r =0.766,P<0.01)和LVEDD(r =0.496,P<0.05)均呈正相关,与LVEF呈负相关(r=-0.634,P<0.01).结论 血浆Gal-3水平与CHF具有一定相关性.血浆Gal-3水平与左室功能和结构的变化相关,表明血浆Gal-3参与了CHF左室重塑的过程.Gal-3对慢性心衰的诊断价值也很高. Objective To study the clinical value of plasma galectin-3 levels in patients with chronic heart failure(CHF).Methods A total of 120 patients with CHF(New York Heart Association functional class Ⅱ-Ⅳ) were enrolled as experiment group,and 40 patients were collected as control group.Plasma Gal-3 and NT-proBNP levels were measured by Elise.Echocardiography was performed to determine left ventricular end-diastolic diameter(LVEDD) and left ventricular ejection fraction(LVEF).The receiver operating characteristic (ROC) curve was used to analyze the diagnostic value of Gal-3 or NT-proBNP in CHF.Results The level of Gal-3 in heart failure group was significantly higher than that in control group[19.5(10.21-22.59)ng/ml vs 5.75(4.66-13.89)ng/ml,P〈0.05],and its level in heart failure group was increasing with the increase of cardiac function grade.The level of the plasma galectin-3 was positively correlated with NT-proBNP(r=0.766,P〈0.01) and left ventricular end diastolic diameter(r=0.496,P〈0.05),and negatively correlated with LVEF(r=-0.634,P〈0.01).Conclusion The level of the plasma galectin-3 had certain relevance with CHF,and it was related to the changes of left ventricular function and structure,which indicated that plasma Gal-3 may have been involved in the process of left ventricular remodeling in CHF.Gal-3 has highly diagnostic value to the chronic heart failure.
