北京市2011-2015年手足口病经济负担的探索性研究

An exploratory study on the economic burden of hand-foot-mouth disease in Beijing, 2011-2015

目的估算北京市因手足口病造成的经济负担。方法利用北京市2011-2015年手足口病监测报告的轻症病例数、重症病例数和死亡病例数，结合既往文献发表的不同类型病例因手足口病而造成的人均经济损失，采用乘法模型，估算每年北京市因手足口病而导致的经济损失。结果2011-2015年，平均每年因手足口病而导致的经济损失合计为5014．3万元，其中直接损失为2829．0万元，间接损失为2185．2万元。轻症病例造成的经济损失为1341．3元／例，其中直接成本为739．8元／例，间接成本为601．5元／例；重症／死亡病例造成的经济损失为12023．7元／例，其中直接成本为9928．1元／例，间接成本为2095．6元／例。结论手足口病对家庭和社会的经济负担均较重，不容忽视，应探索合适的免疫策略和医疗保险策略，以降低其对家庭和社会的经济负担。

Objective To estimate the true incidence rate and economic burden of hand-foot- mouth disease （HFMD） in Beijing. Methods Based on the surveillance data of HFMD-reported number of case, severe case and death, and previously published data on the consultation rate and the economic cost of HFMD, the annual incidence rate and economic burden induced by HFMD was estimated, using multiplier model. Results During the period of 2011 to 2015, the annual economic burden cause by HFMD was estimated at 50.1 million RMB, including a direct cost 28.3 million RMB and an indirect cost 21.9 million RMB. The average cost per episode for mild cases of HFMD was 1 341.3 RMB, including a direct cost 739.8 RMB and an indirect cost 601.5 RMB. The average cost per episode for sever/death cases of HFMD was 12 023.7 RMB, including a direct cost 9 928.1 RMB and an indirect cost 2 095.6 RMB. Conclusions The disease burden induced by HFMD on the family and society was high, and cannot be neglected. Appropriate strategies of vaccination and medical insurance on HFMD are required to be explored to mitigate its burden on the family and society.
