国外慢性病管理模式及对我国慢性病护理管理的启示

随着慢性病患者的数量的逐年上升,慢性病给患者及社会带来一系列问题和挑战.西方国家对慢性病患者的护理,已形成成熟的模式和体系,本文主要对国外慢性病患者的护理模式进行介绍,使广大护理同仁慢性病了解国外慢性病患者的护理管理方法,并对我国慢性病管理提出建议,充分发挥护理人员在慢性病患者管理中的作用,从而改善我国慢性病患者的护理结局.

The number of patients with chronic diseases is increasing year by year,bring a series of chronic problems and challenges to patients and society. Western countries have formed a mature model and system for chronic disease. This paper mainly introduce chronic care models in western country,so as to help nurses and patients in our country to know the management of patients with chronic disease,and then to improve the outcome of patients with chronic disease in our country.
