彩超引导下阻滞C_2～C_5及C_5～T_1神经节治疗头颈肩痛的比较研究

Comparative study on treatment for head neck and shoulder pain with blocking C_2-C_5 and blocking C_5-T_1 ganglion under the guidance of color Doppler ultrasound

目的对比研究彩超引导下阻滞C_2~C_5及C_5~T_1神经节治疗颈肩疼痛的疗效。方法将12例以单侧头痛为主的头颈肩痛患者分为C_2~C_5治疗组和C_5~T_1治疗组;将16例以单侧肩痛、上肢麻木为主的头颈肩痛患者分为C_2~C_5治疗组和C_5~T_1治疗组;在阻滞药物、阻滞方法、阻滞疗程相同情况下对治疗后3 d、1 w、1个月及3个月的VAS疼痛评分及疗效进行对比。结果 C_2~C_5神经阻滞对以头痛为主的头颈肩痛患者短期、长期疗效显著(P<0.05),C_5~T_1神经阻滞对以肩痛、上肢麻木为主的头颈肩痛短期、长期疗效显著(P<0.05)。结论彩超引导下阻滞C_2~C_5神经节治疗头痛为主的头颈肩痛有效,彩超引导下阻滞C_5~T_1神经节治疗肩痛、上肢麻木为主的头颈肩痛有效。

Objective To compare the curative effect of blocking C2-C5 and blocking Cs-T1 ganglion under color Doppler ultrasound in the treatment of head neck and shoulder pain. Methods Twelve patients with head and neck and shoulder pain were divided into C2-C5 treatment group and Cs-T1 treatment group. 16 patients with shoulder pain and upper limb numbness were divided into C2-C5 treatment group and C5-T1 treatment group. The VAS and the curative effect were evaluated 3 days after the treatment, 1 week, 1 month and 3 months with the same block drug,block method and block treatment. Results The C2-Cs nerve blocking had significant short-term and long-term efficacy in head and neck shoulder pain patients predominanted by headache （P〈0.05）,Cs-T1 nerve blocking had significant short-term and long-term efficacy in the patients predominanted by shoulder pain and upper limb numbness （P〈0.05）. Conclusion In the patients with head neck and shoulder pain,C/-C5 nerve blocking in the patients predominanted by headache had short-term and long-term effect significantly （P〈0.0.5）, Cs-T1 nerve block to patients predominanted by shoulder pain,upper limb numbness had short-term and long-term effect significantly （ P〈0.05 ）.
