摘要
延续性护理在肿瘤治疗和康复过程中起着重要作用,本文基于四川省癌症康复会平台,通过建设6个跨学科专业团队,包括肿瘤康复与健康咨询、静脉输液治疗、营养支持、伤口-造口-失禁护理、癌痛姑息治疗、肿瘤放化疗,最终构建了多种延续性医疗护理服务模式,将整合医学和延续护理结合起来。该模式满足了患者延续性护理的需要,促进了医护人员的成长,可推广借鉴。
Transitional care plays an important role in cancer therapy and rehabilitation. This article based on Sichuan cancer rehabilitation association platform, builds 6 interdisciplinary professional teams, including cancer rehabilitation and health advice, intravenous infusion therapy, nutritional support, wound-colostomy-incontinence care, palliative care, cancer radiation and chemotherapy. Eventually, kinds of models of transitional care are established, and combined with integrated medicine. This model meets the needs of transitional care for cancer patients and promotes the growth of the medical staffs, which can be promoted and applied.
