摘要
目的:探讨中指固有动脉血流动力学参数对系统性红斑狼疮(SLE)肾损害的预测价值。方法:对52例临床确诊的SLE患者,将其分为狼疮性肾炎(LN)组和亚临床狼疮性肾炎(SLN)组,选取正常体检50例作为对照组,分别检测各组右手中指固有动脉的收缩期峰值流速(PSV)、舒张期末期流速(EDV)及RI值,并进行两两比较分析。结果:SLE患者中指固有动脉血流信号减少,血流断续,较严重者呈星点状。LN组与SLN组及对照组比较,PSV、EDV降低,RI值升高,差异均有统计学意义(均P<0.05)。SLN组与对照组相比,PSV、EDV差异均无统计学意义(均P>0.05),RI值升高,差异有统计学意义(P<0.05)。结论:中指固有动脉血流频谱在一定程度上反映了SLE对末梢组织和脏器的血流灌注和功能状况的影响。彩色多普勒超声频谱参数可作为反映SLE并发肾损害的早期预测指标。
Objective:To explore the value of color Doppler ultrasound in monitoring the renal impairment in systemic lupus erythematosus (SLE),by testing the middle finger inherent artery. Method:A total of 52 patients were clinically diagnosed with SLE,they were divided into two groups,group lupus nephritis (LN) and group subclinical lupus nephritis (SLN),50 healthy people were selected as control group. The peak systole veloeity(PSV),end diastole veloeity(EDV),resistive index(RI) of the right hand middle finger inherent artery were measured separately and compared between groups. Results:The blood flow signal of middle finger inherent artery decreased in SLE patients,blood flow was intermittent,or with more severe sign of "star or dot in shape". The PSV and EDV values of LN group were lower than that of SLN group and normal gronp,RI was higher,the differences were statistically significant (P〈0.05). There was no significant difference in the PSV,EDV values between the SLE group and the control group (P〉0.05),RI was higher,the difference was statistically significant between two groups(P〈0.05). Conclusion:To some extent,blood flow of middle finger inherent artery would have an effect on the perfusion and function status of peripheral tissue and organ with SLE ,pulsed wave Doppler parameters can be served as early predictors of renal impairment in SLE.
