SIS和聚丙烯网片植入大鼠腹壁后局部组织形态的中长期观察

摘要 目的:观察SIS和聚丙烯网片植入大鼠腹壁后局部组织形态中长期的改变和区别。方法:60只成年SD大鼠随机分为3组(每组20只),在大鼠腹壁形成2cm×2cm人工缺损,将聚丙烯合成网片(A组)、SIS生物网片(B组)植入大鼠腹壁后,与无网片植入组(C组)对比。术后3、9、15、21月,观察网片植入局部宏观和微观(电镜下)的形态学的改变。结果:术后SD大鼠减损例数:A组4例,B组3例,C组3例,共10例,三组间无差异(P>0.05)。随着时间延长,术后各组均有不同程度的修复。聚丙烯网片组增生明显,但异物感最强,腹壁僵硬,触摸时有明显的沙粒感;SIS网片组柔软、舒适,但有较为明显的薄弱感;对照组缝合处与周围组织触感相同,偶及疤痕或结节。电镜下:聚丙烯网片组成纤维细胞增殖更为活跃,数量也较多,组织较为紧密,胶原纤维增生多且致密,胶原纤维直径粗,但异物界限稍有模糊,多层细胞管壁的较大血管,管腔逐渐增大,管壁逐渐增厚;SIS生物网片组较为稀疏,散乱,细胞数较少,胶原纤维增生效果最差,断裂模糊,组织内异物界限逐渐模糊、质地变薄变淡,有消失趋势,血管增生最少,管壁最薄,管腔也最小,周围组织较少;对照组成纤维细胞增生活跃,胶原纤维致密,血管增生较多,未见明显异物。结论:聚丙烯网片组组织增生明显,但异物感强;SIS网片组织相容性良好,但增生不佳,提供张力的结缔组织较为薄弱。开发同时具有各自优点的网片将是继续研究和应用盆底生物网片的方向。
