化瘀通络汤联合低分子肝素钙治疗进展性脑梗死临床研究

Huayu Tongluo Decoction Combined with Low Molecular Weight Heparin Calcium in Treatment of Progressive Cerebral Infarction

目的:探究化瘀通络汤联合低分子肝素钙治疗进展性脑梗死的临床疗效。方法:将进展性脑梗死患者116例,随机分为治疗组和对照组。两组均给予降血脂、降血糖、降血压、抗血小板等常规治疗,同时皮下注射0.4 m L低分子肝素钙,每日2次。治疗组在观察组基础上加服化瘀通络汤,连续治疗7 d,统计分析两组治疗后临床有效率、神经功能缺损程度及不良反应等。结果:治疗组有效率为96.6%,对照组有效率为84.5%,两组比较差异有统计学意义(P<0.05)。两组患者治疗后神经功能缺损程度及日常生活能力均较治疗前改善明显(P<0.05),且治疗组明显优于对照组(P<0.05),两组患者治疗过程中均未发现药物相关不良反应。结论:化瘀通络汤联合低分子肝素钙可显著改善进展性脑梗死患者神经功能和生活能力,疗效显著,同时无明显不良反应。

Objective ：To observe the clinical efficacy and safety of Huayu Tongluo Decoction combined with low molecular weight heparin calcium in treatment of progressive cerebral infarction. Methods ：116 cases of progressive cerebral infarction patients in Re- habilitation Department of Zhongshan Hospital from June 2014 to May 2015 were randomly divided into the treatment group （ 58 cases） and control group （58 cases）. Both groups received conventional treatment including controlling blood sugar, blood pres- sure and blood fat, anti-platelet therapy. And they were also treated with 0.4mL subcutaneous low molecular weight heparin calci- um on the basis of the conventional therapy,2 times a day, for 7 days. Besides, the treatment group was treated with Huayu Tongluo Decoction. The therapeutic effect,the activities of daily living （ADL） and the total effective rate of both groups were measured be- fore and after treatment. Results ： The total effective rate of the treatment group （ 96.6% ） was significantly higher that that （84. 5% ） of the control group （P 〈 0.05 ）. The NIHSS score and ADL score of two groups had no significant difference before treatment （P 〉 0.05）. The NIHSS score and ADL score of two groups after treatment were superior to those before treatment re- spectively （P 〈 0.05 ）. The NIHSS score and ADL score of the treatment group were superior to those of the control group respec- tively after treatment （P 〈 0.05 ）. No drug-related adverse reactions were found during the treatment. Conclusion ： Huayu Tongluo Decoction combined with low molecular weight heparin calcium could improve nerve function and living ability of progressive cere- bral infarction without increasing any risk of adverse reactions.
